RFP 1152: Health Benefits Consultant

The Town of Bloomfield invites proposals for the attached RFP on & after 5/14/2025. IRS Form W-9, Request for Taxpayer Identification Number & Certification must be completed & submitted with your proposal. Please conform to our format request as closely as possible. The Town may accept responses that have exceptions to any requirement in the RFP that are clearly delineated in a separate attachment to the submission. The Town reserves the right to terminate this RFP process at any time with or without notice to the prospective Respondent. All proposals must be received in the Purchasing Dept. by Wednesday, 6/11/2025, at 1pm EST. 1 Original plus 2 read only paper copies, & 1 copy on a thumb drive, shall be submitted to : Town of Bloomfield Purchasing Dept, Town Hall, 2nd Flr, 800 Bloomfield Ave., Bloomfield, CT 06002. Proposals shall be sealed, bearing on the outside the firm's name & address & plainly marked RFP #1152: HEALTH BENEFITS CONSULTANT.
