I. Introduction
The City of Bangor (City) Department of Public Health and Community Services (BPHCS)
requests proposals for the furnishing and delivery of 4 mg nasal naloxone, 3 mg nasal
naloxone, and 0.4 mg/1ml intramuscular injectable naloxone. Proposers may include one (1)
or more products in the proposal; only one (1) submission is needed.
Bangor Public Health and Community Services’ naloxone distribution program is part of the
State of Maine’s Naloxone Distribution Initiative, a statewide effort to make naloxone widely
accessible to the public. Purchases are subject to the availability of grant funds.
II. General Information
Available at the following web address: www.bangormaine.gov/proposals on the City’s
website. By submitting a response to this solicitation, the Proposer accepts the responsibility
for downloading, reading, and bidding by the terms and conditions set forth in the City’s
“General Information for Vendors.”
In your proposal, please specify whether you currently have, or are in the process of
developing, a domestic violence policy. If you do not have such a policy, let us know if you
would like to receive a copy of the City of Bangor's policy as a reference.
III. Submission
For consideration, submit the Bid Form (pages 7-10), Appendix B, Appendix C, and Appendix
D electronically or in an envelope via submission methods listed below. The envelope or
subject line must be clearly marked as "Proposal No. P25-50: Naloxone Kits."
Submissions must be received by 2:00 P.M. on Wednesday, April 23, 2025.

B. Hand Delivery to 262 Harlow Street, Bangor, ME (back entrance of building at City of
Bangor entrance); or
C. US Post Office addressed to 73 Harlow Street, Purchasing Department, Bangor, ME
04401 (items must be received by the deadline); or
D. All Other Delivery Services to City of Bangor: Purchasing Department, 262 Harlow
Street, Bangor, ME 04401 (proposal must be received by the deadlines).
All submissions should reference “Proposal No. P25-50: Naloxone Kits.” Proposals will be
publicly opened at the time stated above in the temporary Council Chambers, 262 Harlow
Street, Bangor, Maine (see Appendix A - Meeting Location Map).
A tabulation of all received proposals will be posted on the City's website by 4:30 P.M. on the
opening date. Visit www.bangormaine.gov/bids for results. The "Show
Closed/Awarded/Cancelled Bids" box must be checked to see the proposal information.
IV. Questions
Any questions must be directed in writing to bids@bangormaine.gov no later than 4:30 P.M.,
Friday, April 4, 2025.
The City will provide a response by 4:30 P.M. on Friday, April 11, 2025. This response will
be in the form of an addendum, accessible on the City's website. Notifications will be sent to
those who have signed up to receive notifications at www.bangormaine.gov/bids through
NotifyME. Notifications will be sent when new addenda are released.
V. Late Proposals
It is the responsibility of the Proposer(s) to see that their proposals have sufficient time to be
received by the Purchasing Department before the submittal deadline. Any proposal, portion
of a proposal, or requested proposal revision received at the City Purchasing Department after
the time and date specified will be returned to the Proposer unopened or, if by electronic
submission, deleted without being opened.
VI. Withdrawal of Proposals
No Proposer may withdraw their proposal for a period of ninety (90) days from the date of
opening. All proposals shall be subject to acceptance by the City during this period.
To withdraw a proposal prior to the opening, the Proposer shall request the withdrawal in
writing. All costs associated with the withdrawal (i.e., mailing fees) will be borne by the
Proposer.
VII. Rejection
The City reserves the right to reject any proposals, waive any informalities or defects in
proposals, or accept a higher-cost proposal if it is deemed to be in the best interest of the City.
The City also reserves the right to request clarification of any details from the successful
Proposer.

VIII. Information for Proposers
A. Tax Exclusion: All Federal and State taxes must be excluded from the proposal price.
Upon request, a tax exemption certificate for the City of Bangor shall be furnished to the
successful Proposer.
B. Bid Form, Appendix B, Appendix C, and Appendix D: The Bid Form (pages 7-10),
Appendix B, Appendix C, and Appendix D must be completed and returned with the
proposal to be considered responsive.
C. Required Insurance: Proposers are required to maintain the following insurance policies
throughout the life of the project and name the City of Bangor as an additional insured
where applicable and may be required to provide proof:
1. Worker’s Compensation
Insurance
2. Employer’s Liability Insurance
3. Commercial & General Liability
Statutory
$500,000 each accident
$500,000 disease – policy unit
$500,000 disease – each employee
$1,000,000 combined single limit
$2,000,000 aggregate
IX. Requirements and Preferred Criteria
All proposals must include the name of the manufacturer, trade name, brand name, and all
product specifications. The product being offered must have be approved by the Food and
Drug Administration (FDA).
A. 4 mg Nasal Naloxone Kits
1. Requirements
a. Distribution Capacity:
i. Current capacity to distribute 2,500+ units of one (1)-box, two (2)-dose, 4 mg
nasal naloxone spray per month.
ii. Potential for expanded capacity in the future.
b. Ordering and Shipping:
i. Ability to invoice BPHCS for payment.
ii. Ability to ship product within one (1) week of order placement.
iii. Ability to ship orders directly to multiple recipients in multiple locations across
Maine.
iv. Free shipping on all orders.
v. Real-time shipping/tracking information for recipients.

c. Customer Support: Technical assistance and direct contact with customer
service representatives, as needed.
2. Preferred Criteria
a. No minimum quantity required for order placement.
b. Access to marketing and free educational materials upon request.
c. Access to demo testers by request.
d. Online multi-user ordering system.
e. Online multi-user ordering system with enhanced features such as the ability to
manage multiple funding streams and generate reports by timeframe, zip code,
agency, type of agency, or county.
f. Non-profit organization.
B. 3 mg Nasal Naloxone Kits
1. Requirements
a. Distribution Capacity:
i. Current capacity to distribute 750+ units of one (1)-box, two (2)-dose, 3 mg
nasal naloxone spray per month.
ii. Potential for expanded capacity in the future.
b. Ordering and Shipping:
i. Ability to invoice to (BPHCS) via purchase order.
ii. Ability to ship product within one (1) week of order placement.
iii. Free shipping on all orders.
iv. Real-time shipping/tracking information for recipients.
c. Customer Support: Technical assistance and direct contact with customer
service representatives, as needed.
2. Preferred Criteria
a. No minimum quantity required for order placement.
b. Access to demo testers by request.
c. Access to marketing and free educational materials upon request.
d. Non-profit organization.

C. 0.4 mg/1ml Intramuscular Injectable Naloxone (IM) Kits
1. Requirements
a. Distribution Capacity:
i. Current capacity to distribute 500 IM kits (two (2)-dose 0.4 mg/1ml
intramuscular vials and two (2) syringes) per month.
ii. Potential for expanded capacity in the future.
b. Ordering and Shipping:
i. Ability to invoice BPHCS for payment.
ii. Ability to ship product within one (1) week of order placement.
iii. Free shipping on all orders.
iv. Real-time shipping/tracking information for recipients.
c. Customer Support: Technical assistance and direct contact with customer
service representatives, as needed.
2. Preferred Criteria
a. No minimum quantity required for order placement.
b. Access to marketing and free educational materials upon request.
c. Non-profit organization.
X. Contract Award
The City will select the proposal deemed most advantageous, appropriate, and beneficial to
the City. The evaluation of proposals and determination of the award will be at the discretion
of the City, and its judgment shall be final and without right of recourse by any Proposer.
XI. Equal Opportunity and Non-Discrimination
The successful Proposer agrees not to discriminate against any employee or applicant for
employment because of age, race, color, religion, sex, or national origin. The successful
Proposer shall comply with applicable laws, executive orders, and regulations concerning
nondiscrimination in employment, including the Equal Opportunity Clause of Section 202 of
Executive Order 11246, as amended, which is hereby incorporated by reference. Equal
Employment Opportunity (Appendix D) must be completed and included with proposal.
XII. Health Related Laws and Regulation
All services provided under this contract shall be provided in accordance with all applicable
Federal, State, and local laws. The successful Proposer must not be currently excluded or
