SCI-PHOENIX IS SOLICITING QUOTES FOR THE LABOR, EQUIPMENT, MATERIALS, AND REPLACEMENT COMPONENTS NECESSARY TO REPLACE THE AUGERS AND SCREENS AT THE WASTEWATER PLANT LOCATED AT THE STATE CORRECTIONAL INSTITUTION AT PHOENIX, 1200 MOKYCHIC DRIVE, COLLEGEVILLE, PA 19426 IN ACCORDANCE WITH THE STATEMENT OF WORK. THERE IS A MANDATORY SITE VISIT SCHEDULED AT SCI PHOENIX FOR VENDORS THAT HAVE NOT PREVIOUSLY ATTENDED A SITE VISIT TO VIEW THE EQUIPMENT ON NOVEMBER 18, 2024 AT 9:00am. ATTACHMENTS ARE INCLUDED BELOW TO INCLUDE: 1. INSTRUCTIONS, 2. CLEARANCE CHECK FORM, 3. CONTRACTOR TOOL INVENTORY LIST. 4. CONTRACTOR PREA TRAINING FORM. ALL CLEARANCE FORMS MUST BE COMPLETED AND SUBMITTED TO SAKURTZ@PA.GOV BY COB ON MONDAY, NOVEMBER 11. PAPERWORK & CLEARANCES MUST BE APPROVED PRIOR TO SITE VISIT.
