Description: The purpose of this RFP is to compare the services, fees and offerings of several vendors for medical services. While this is a due diligence exercise, the Village of Garden City is not opposed to considering a move if another vendor offers better value and better service for the Village and service to its employees. Your Electronic Response should be emailed to Courtney Rosenblatt at crosenblatt@gardencityny.net on or before 4:00 p.m. EST on Thursday, December 5, 2024. Your electronic answers should be as brief and specific as possible. You must complete the Vendor RFP Questionnaire in excel format as directed. Do not change the format of this file. The returned file should not be locked unless a password is provided. Your Hard Copy Response with attachments and samples should be sent to the following location on or before 4:00 p.m. EST on Thursday, December 5, 2024:
