Pest Control Service to be performed at:
Alabama Department of Rehabilitation Services
4104 HWY 31 S , VRS Building (#2) and BEP/ADT Shop (#17), Decatur, AL 35603

Total square footage is 22,625 sq ft.

The supplier shall furnish all labor, supervision, transportation, tools, materials and supplies necessary to perform pest control services in accordance with the guidelines established by Federal, State and Local Government laws, rules and regulations. All applicable EPA regulations and rules shall be followed (www.epa.gov).

Suppliers to supply each facility with a Pest Control Log Book, MSDS documents for all products utilized, copy of service report for each visit, contact for Poison and Pest Control Hotline, and a Pest Sighting page that allows appropriate personnel to document known pest control issues.

Each supplier to possess an Alabama Pest Control License or Pesticide Applicator License from the Alabama Dept of Agriculture and Industries.  Supplier must have a minimum of $100,000.00 liability coverage with $50,000.00 for each occurrence and provide proof of workman’s compensation insurance sufficient to satisfy all legal requirements of the State of Alabama if employees in number are 5 or greater.  Supplier to include all documentation requested when submitting bid.

PEST CONTROL SERVICE
During each scheduled visit to the facility, the contractor shall conduct inspections for potential pest infestations and take immediate action to eradicate pests as they are encountered.

Definition of Work:

1. Inspections of all kitchen/breakroom, offices (according to approved schedule), restrooms, janitorial closets, conference rooms, BEP/ADT Garage and Warehouse, etc.
2. Inspection of the exterior perimeter of each facility.
3. Judicious applications of pesticides shall be made depending on the findings of the inspection such as the past or current presence of pest or conditions that render a specific area to be reasonable susceptible to pest activity.
4. Monthly treatment of the building exterior, particularly around all potential points of entry for pest such as windows, doors, vents, and minor structural defects, etc.

Technicians shall check in at the receptionist office of the ADRS Facility with a photo ID and log in and out of the supplier provided Pest Control Logbook kept at the ADRS office.  Pest Control times will be between the hours of 6:30 am – 3:00 pm Monday thru Friday.  Service date to be the same date each month and must be agreed upon by awarded supplier and ADRS Field Supervisor.

A site visit is scheduled for Monday, August 24, 2026, at 10:00 am.  Suppliers attending site visit must sign the sign-in sheet for bids to be considered.  The contact for the site visit is Nancy Adams/Chrisy Kochie.  No subcontracting services and supplier must reside in the State of Alabama.
