Ohio Department of Health-58664-Bureau of Maternal Child & Family Health-SFY26-27-School-Based Health Center Technical Assistance

The Bureau of Maternal Child & Family Health (BMCFH) is seeking proposals from qualified vendors to assist in collecting information on School Based Health Centers (SBHCs), provide technical assistance (TA) and support to DOH funded and non-DOH funded SBHC in Ohio.

Offerors will need to review the attached RFP DOH58664 SRC0000034288. Each Offeror must submit a technical proposal and cost proposal as part of its total Proposal before the opening time on the Proposal due date. Further, the Offeror must title the electronic files with “DOH RFP #SRC0000034288/DOH58664 - Technical Proposal” and "DOH RFP #SRC0000034288/DOH58664 - Cost Proposal” as appropriate. We also ask bidders to complete the attached cost template form.
Please note there is not a page minimum or maximum for the technical proposal and bidders should refer to technical evaluation criterion on the RFP document for guidance.
