Summary

Respirators may be required for use by ODH employees during routine  job performance.  If  respirators are required as part of the PPE, ODH employees must be provided with respirator fit-testing per OSHA, 29 CFR 1910.134.  Fit testing must be performed by trained individuals who can make the determination of medical clearance and respiratory fit testing in locations accessible by ODH employees.

Awardee must have the ability to perform the three components of the fit testing through quotes  as follows: review of the medical clearance form,  employee physical if results of clearance warrant exam, respiratory fit test.

Process

Offerors will need to review the attached RFP ODH #SRC0000023231. Each Offeror must submit a technical proposal and cost proposal as part of its total Proposal before the opening time on the Proposal due date. Further, the Offeror must title the electronic files with “ODH RFP #SRC0000023231/DOH55500 - Technical Proposal” and "ODH RFP #SRC0000023231/DOH55500 - - Cost Proposal” as appropriate.
