The Contractor shall deliver mobile medical Supplies, delivery and training necessary to allow the Connecticut Early Detection and Prevention Program (CDEPP) an integrated program consisting of the CT WISEWOMAN Program and CT Breast & Cervical Cancer Early Detection Program provider sites throughout the State of Connecticut and their associated intake centers to carry out Minimum Data Elements required by the Centers for Disease and Control and Prevention including height, weight, blood pressure, body mass index blood work to include: total cholesterol, high-density lipoprotein, low-density lipoprotein, triglyceride levels, fasting glucose & Hemoglobin A1C.
