The County of Santa Cruz invites sealed proposals from fully licensed, insured, bonded, and certified vendors to furnish all labor, tools, equipment, and incidentals required to provide CalAIM Justice-Involved Pre-Release Medi-Cal Application Outreach . This support is particularly focused on Justice-Involved (JI) individuals who are released from incarceration quickly, are housed in juvenile justice facilities, or require additional assistance with their applications. This contract is funded through the California Department of Health Care Services (DHCS) under the California Advancing and Innovating Medi-Cal (CalAIM) Providing Access and Transformation (PATH) Justice-Involved Capacity Building Round 2 initiative. It will establish a joint agreement between the selected organization and the County of Santa Cruz Human Services Department (HSD), the Probation Department (PRB), and the Sheriff’s Office (SHF) with the goal of enhancing coordination and improving access to essential services for individuals transitioning out of jail or juvenile hall. These funds can be used to strengthen collaboration with Justice Agencies and ensure programmatic decisions are made strategically while using available data for the County of Santa Cruz.
According to CalAIM, a justice-involved individual is defined as “an individual who is currently or was formerly incarcerated within the past twelve months."
Services provided through this contract will support outcomes aligned with CalAIM’s Enhanced Care Management (ECM) and Community Supports (CS) programs specifically targeting the "Population of Focus"—adults and youth transitioning from incarceration, including:
Additionally, this contract will assist in the development of the County-wide Pre-Release Medi-Cal Application Process and a shared data tracking tool.
Available funding for this project is not to exceed $747,000 for the original contract term.
