STATEMENT OF WORK (SOW)








  1. BACKGROUND





CGC Hollyhock recently had a discrepancy of water intrusion inside the cabinet of their Welin Lambie Dual Point Davit. The discrepancy required a replacement main inverter to correct the issues. However, the crew was unable to determine the root cause or potential of cascading issues from the casualty. CGC Hollyhock requires a Welin Lambie certified Tech-Rep to go through and reevaluate and test the system. In addition to recertifying the system, CTR must replace the adjustable elbow fitting outlined below which cause a major hydraulic leak due to a failed O-Ring.

        1. PROPOSAL TO PROVIDE (1) SENIOR FIELD SERVICE TECHNICIAN WORKING (10) HOURS PER DAY FOR (1) DAY - TECH REP TO ASSIST WITH PARTS INSTALL, CONDUCT DAVIT ASSESMENT, AND WITNESS DAVIT OPERATIONS. ANY NOTED DISCREPANCIES WILL BE REPORTED VIA CFR PROCESS.

  1. 4.0 ADMINISTRATION.







4.1 CONTRACTOR PERSONELL





4.1.1 Qualified Personnel.



The Contractor shall provide qualified personnel and subject matter experts to perform all required tasks specified in this SOW.





4.1.2 Employee Conduct.



Contractor employees shall present a professional appearance, and their conduct shall not reflect discredit upon the U.S. Government, and any involved Departments or Agencies. Contractor employees shall comply with all applicable Government regulations, policies, and procedures (e.g., fire, safety, sanitation, environmental protection, security, “off limits” areas, wearing of parts of military uniforms, and possession of weapons) when visiting or working at Government facilities.





4.1.3 Conflict of Interest.



The Contractor shall not employ any person who is an employee of the U.S. Government if that employment would, or would appear to cause, a conflict of interest. The Contractor shall take appropriate precautions as delineated in the Federal Acquisition Regulation (FAR) to preclude actual or apparent conflicts of interest.





4.1.4 Base Access



U.S Coast Guard Yard requires registration for guest access from contractors. Contractor must provide a valid I.D and name to the USCG POC’s no later than one week prior to the commencement of the contract to gain base access.





4.2 PERIOD OF PERFORMANCE.





One day on site between 18JULY-01AUG
