Summary

The objective of the project is to provide evaluation services for two health promotion programs:
•    State Physical Activity and Nutrition (SPAN) program
•    Creating Healthy Communities (CHC) program

PROJECT 1 - STATE PHYSICAL ACTIVITY AND NUTRITION (SPAN) PROGRAM

Execute the current evaluation plan for the SPAN Program. The plan includes both process and outcome evaluation to assess program approach, effectiveness, efficiency, sustainability, and impact. Quantitative and qualitative process and outcome performance measures will be used to set baselines and targets and monitor progress toward desired outcomes. Evaluation activities will include developing, selecting, evaluating, and/or providing recommendations regarding surveys, inventories, or other data/information collection instruments; conducting focus groups and interviews; analyzing data; developing evaluation dissemination products; and developing progress/performance reports and sustainability plans. The development of future evaluation plans upon receipt of new grant funds beyond September 30, 2028 will be determined in consultation with ODH for the duration of the contract period.


PROJECT 2 - CREATING HEALTHY COMMUNITIES (CHC) PROGRAM

Execute the current evaluation plan for the CHC program to address the impact, reach, and sustainability of CHC activities in increasing access to healthy foods and physical activity opportunities. The plan will include both process and outcome evaluation to assess program approach, effectiveness, efficiency, sustainability, and impact of strategies and activities. Evaluation activities will include developing, selecting, evaluating, and/or providing recommendations regarding surveys, inventories, or other data/information collection instruments; conducting focus groups and interviews; analyzing data; developing evaluation dissemination products; and developing progress/performance reports and sustainability plans. The development of an evaluation plan for future CHC granny cycles beyond September 30, 2029 will be determined in consultation with ODH for the duration of the contract period.

The Ohio Department of Health (ODH) is soliciting proposals for professional services.

1.6. Project Background: The State Physical Activity and Nutrition (SPAN) Program, funded through
a cooperative agreement from the Centers for Disease Control and Prevention (CDC), is a
collaboration between the Bureau of Health Improvement and Wellness (BHIW) and the Bureau
of Maternal Child and Family Health (BMCFH) programs at the Ohio Department of Health. Ohio
is one of 17 states to receive SPAN funds. SPAN’s goal is to make healthy eating and active living
more accessible. SPAN recipients work on evidenced based strategies for: making healthy food
choices easier through the promotion of food service and nutrition guidelines and the expansion
of fruit and vegetable voucher incentives and produce prescription programs; safe and accessible
physical activity; continuity of care in breastfeeding support; and healthy eating and active living
in early care and education. These strategies address health disparities related to poor nutrition,
physical inactivity, and/or obesity. SPAN operates on a five-year cycle, with the current
cooperative agreement running from October 1, 2023-September 30, 2028.
The Creating Healthy Communities (CHC) Program at the Ohio Department of Health has a vision
of ‘vibrant Ohio communities where everyone has access to healthy food and opportunities for
active living.’ This is achieved through CHC’s mission of ‘activating community-led solutions to
create sustainable change in policies, places, and population health. CHC serves 31 counties
through their local health departments with a strong focus on community engagement. Target
populations include those with the lowest income and education, those living in underserved
areas, and those with limited or no access to healthy food and physical activity opportunities.

CHC operates on a five-year grant cycle, with the current cycle running from October 1, 2024-
September 30, 2029. Current CHC evaluation efforts include a principles-focused evaluation
approach.

1.8. Projected Estimated Budget: $175,000.00 Not to Exceed for the project period as listed in
section 1.10
1.9. Project Award: One (1) Award.
1.10. Project Period: 07/01/2026-06/30/2027
ODH reserves the right to execute multiple agreements with awarded provider to fulfill the
entire project period, subject to and contingent on the discretionary decision of the Ohio
General Assembly to appropriate funds (if needed) for the biennium, satisfactory performance
of the awarded providers and the needs of the Ohio Department of Health
1.11. Agreement Term: 07/01/2026-6/30/2027 with up to four (4) years of additional Renewal Terms
may not exceed after 06/30/2031.
1.12. Renewal Terms: 48-months optional renewal period(s) dependent upon the receipt and
approval of funds to ODH for this project, this agreement may be renewed into four (4) 12-month
terms.
Renewal Optional Year 1: 07/01/2027-06/30/2028
Renewal Optional Year 2: 07/01/2028-06/30/2029
Renewal Optional Year 3: 07/01/2029-06/30/2030
Renewal Optional Year 3: 07/01/2030-06/30/2031
At the sole option of ODH, ODH may extend this Contract past the initial Agreement Term for a
period of ninety (90) days. Renewal terms may be exercised by mutual agreement between the
Contractor and ODH. The cumulative time of all mutual renewals may not exceed four (4)
additional years and are subject to and are contingent upon the discretionary decision of the Ohio
General Assembly to appropriate funds for this project in each new biennium. If any renewal is
exercised, a new contract will be issued at the beginning of the new biennium. ODH may evaluate
whether a renewal is appropriate considering the satisfactory performance of the Contractor and
the future and continuing needs of ODH’s Programs.

2. PROJECT REQUIREMENTS: Offeror(s) must meet the following mandatory requirements to be
considered for evaluation.
2.1. Contractor Experience Requirement:
2.1.1.Offeror(s) must have Relevant experience of two (2) planned project personnel in developing
and implementing evaluation plans of a similar scope and size that include process and
outcome evaluation.
2.1.2. Offeror must possess relevant experience and expertise by documenting successful
completion of a minimum of two (2) projects similar in size and scope within the past five
(5) years.
2.1.3. Offeror has relevant experience of two (2) planned project personnel in
developing/conducting surveys, focus groups, interviews and other data collection activities,
and analyzing and evaluating data collected.
2.1.4. Offeror has relevant experience and formal education of planned project manager and key
project personnel with projects of similar size and scope within past five (5) years.
2.2. Candidate Expertise Requirement:
2.2.1.Offeror(s) must have expertise in designing and implementing surveys, inventories and other
data/information collection systems.
2.2.2. Offeror(s) must have expertise conducting focus groups and interviews to inform program
development, implementation and improvement.
2.2.3. Offeror(s) must have expertise analyzing data and information gathered from surveys,
inventories other data collection instruments, focus groups and interviews.
2.2.4. Offeror(s) must have expertise preparing evaluation and performance measurement plans
and progress reports.
2.2.5. Offeror(s) must have expertise preparing and disseminating reports of evaluation findings.
2.2.6. Offeror(s) must have expertise using evaluation findings for continuous program
improvement and to inform program sustainability.

3. SCOPE OF WORK AND DELIVERABLES:

3.1. Scope of Work: PROJECT 1 - STATE PHYSICAL ACTIVITY AND NUTRITION (SPAN) PROGRAM
3.1.1
Prepare for and participate in monthly conference calls with CDC and SPAN program staff by preparing
and sending an agenda in advance of the call and providing a list of work completed in the previous
month and any questions for SPAN staff and CDC. Correspond by email or phone with the ODH Evaluation
Supervisor on at least a monthly basis regarding assignments and related progress.
3.1.2
Refine and implement a comprehensive evaluation and performance measurement plan for the SPAN
program that includes both process and outcome evaluation to determine the impact of CDC funding on
nutrition and physical activity outcomes. The evaluation framework should include plans for monitoring
strategy implementation, evaluation methods to determine effectiveness of selected interventions at
achieving expected outcomes and plans to collect data to determine factors that may influence target
populations, specific settings and scalability of program interventions.
3.1.3
Participate in one (1) in-person site visit per year in Columbus, OH or other designated location with SPAN
Program staff. Prepare an agenda in advance of the site visit that includes topics, timelines, and
participants for each session. Topics may include a review of contract deliverables completed, review of
future deliverables and deadlines, development/refinement of the Evaluation and Performance
Measurement Plan, and interviews with key program staff and contractors. Provide a written summary
of findings from the meeting that would inform the SPAN program. Or, if required, participate in CDC-
sponsored SPAN meetings in Atlanta, Georgia or designated location. Participate in evaluation-related
sessions. Provide a written summary of findings from the meeting that would inform the SPAN program.
3.1.4
Complete semi-annual and annual progress/performance reports on the results of the evaluation and
performance measurement plan. The reports should address evaluation activities conducted during the
previous six months, including deliverables completed, evaluation results, successes/challenges, and
recommendations to improve the quality and sustainability of the program. The report should also
outline evaluation activities to be conducted during the next six months of the project period.
3.1.5
Develop and/or provide written feedback on surveys, inventories, or other data/information collection
instruments. Work may include, but is not limited to, developing electronic survey tools, spreadsheets,
databases, and other platforms for data collection and reviewing/editing existing instruments. Surveys,
inventories, and other data/information collection instruments may be used to assess facilitators,
barriers, successes, and challenges in implementing program strategies and activities.
3.1.6
Implement qualitative data collection procedures including focus groups and interviews. Work may
include, but is not limited to, developing focus group and interview questionnaires, conducting focus
groups and interviews in-person or via conference call, and recording/transcribing responses. Qualitative
data collection procedures may be used to assess facilitators, barriers, successes, and challenges in
implementing program strategies and activities.
3.1.7
Analyze and evaluate data and information collected from surveys and other data collection systems,
focus groups, and interviews, and provide a written summary of results. Work may include, but is not
