Dental Services - State Training School
RFP Purpose.
The Iowa Department of Human Services (“Agency”) State Training School (STS) located in Eldora, Iowa, is
seeking qualified bidders to provide oral exams, dental treatment, preventative services and tooth extractions to
the students of the State Training School.
Duration of Contract.
The Agency anticipates executing a contract that will have an initial 2-year contract term with the ability to extend
the contract for 2 additional 2-year terms. The Agency will have the sole discretion to extend the contract.
Procurement Timetable
There are no exceptions to any deadlines for the bidder; however, the Agency reserves the right to change the dates.

Dental Services - State Training School
Section 1 Background and Scope of Work
1.1 Background.
In accordance with Iowa Code, Chapters 218 and 233A, and 441 Iowa Administrative Code §103, the State
Training School at Eldora is to provide a continuum of supervision and rehabilitation programs that meet the
needs of males adjudicated delinquent, in a manner consistent with public safety. These services and programs
individualize treatment and control the offender for his own benefit and the protection of society. The facility
provides a range of services that are geared toward
Providing a structured treatment setting for males between the ages of 12 to 18;
Providing the most appropriate setting while protecting communities;
Developing individual care plans, which provide a diversity of innovative and effective programs within
the framework of professional correctional standards, legislative intent and available resources;
Providing a comprehensive education program for youth; and
Facilitating the transition of the young man back to his community.
The State Training School establishes policies and procedures in accordance with standards set forth by the
American Correctional Association (ACA). The State Training School has been awarded Accreditation by the
ACA in 2012, 2015 and 2018.
1.2 RFP General Definitions.
Definitions in this section correspond with capitalized terms in the RFP.
“Agency” means the Iowa Department of Human Services.
“Bid Proposal” or “Proposal” means the bidder’s proposal submitted in response to the RFP.
“Contractor” means the bidder who enters into a Contract as a result of this Solicitation.
“Deliverables” means all of the services, goods, products, work, work product, data (including data collected on
behalf of the Agency), items, materials and property to be created, developed, produced, delivered, performed, or
provided by or on behalf of, or made available through, the Contractor (or any agent, contractor or subcontractor
of the Contractor) in connection with any contract resulting from this RFP.
“Invoice” means a Contractor’s claim for payment. At the Agency’s discretion, claims may be submitted on an
original invoice from the Contractor or may be submitted on a claim form accepted by the Agency, such as a
General Accounting Expenditure (GAX) form.
1.3 Scope of Work.
1.3.1 Deliverables.
The Contractor shall provide the following:
1) The contractor shall provide professional Dental services to include oral examinations and treatment,
preventative services and tooth extractions as needed.
2) The Contractor shall be a state licensed dental care provider and comply with all appropriate regulations
and provide proof of licensure and insurance.
3) The Contractor shall be responsible for providing a dental assistant if deemed necessary by contractor.
4) The Contractor shall be available to provide emergency services after normal office hours and on
weekends by providing the department with a telephone number for after-hour emergencies.
5) The Contractor will ensure these deliverables are covered during his/her absence due to vacation or
extended illness.

6) The Contractor shall comply with all applicable federal and state laws, regulations and requirements
regarding the Prison Rape Elimination Act (PREA).
Contractor using on-site Facility.
1) The Contractor shall normally provide dental services to the students at the State Training School on-site
at the Arlene Dayhoff Education Complex clinic. The Contractor shall provide a minimum of three (3) visits per
month not to exceed four (4). The professional dental services will be for up to eight hours, one day per week or
as scheduled by the contract manager not to exceed 14 days between visits.
2) The Contractor shall maintain an inventory of supplies and equipment issued by the State Training School
to provide dental services. As required by State Training School procedures and state regulatory requirements,
equipment used for dental services shall be sanitized, inventoried, and securely stored at the end of each service
day.
3) The Contractor shall request approval from the contract manager or designee for any changes in working
days and the contract manager shall monitor and record all dates of service

2) The Contractor will be provided with a list of the students who are scheduled for dental care services for
each scheduled day of service.
3) The Contractor shall be provided all appropriate dental care supplies and equipment. Requests for
supplies and equipment shall be through the contract manager in accordance with State Training School
purchasing policy and procedures.
4) Ensure the contract understands and complies with all Federal and State laws, regulations and
requirements regarding the Prison Rape Elimination Act (PREA).
1.3.2 Performance Measures.
The Contractor shall submit a monthly invoice with the completed Code on Dental Procedures and Nomenclature
(CDT) procedures listed, costs per CDT and the number of students to whom the dental services were given. The
Contractor shall keep an inventory of all equipment and supplies kept in the dental office. In accordance with
policy and procedures, State Training School may audit inventories, verify sanitization of the equipment and
secured storage at any time.
1.3.3 Contract Payment Methodology.
In accordance with the payment terms outlined in this section and the Contractor’s completion of the scope of
work as set forth in this Contract, the Contractor will be compensated in the following manner:
Performed at the STS: Dental provider performs services at the dental office on the STS, the service provider will
be paid per $200 per hour, with a minimum of $400 per day, whichever is greater for each scheduled visit. The
service provider shall The Service Provider will be paid for the services described not to exceed $48,000 annually.
The Service Provider shall submit, on a monthly basis, an invoice for services rendered in accordance with this
Contract. Payment shall be contingent on the Service Provider performing the mandatory services set forth,
meeting performance measures listed, and submitting a detailed monthly invoice.

2.2 Restriction on Bidder Communication.
From the issue date of this RFP until announcement of the successful bidder, the Issuing Officer is the point of
contact regarding the RFP. There may be no communication regarding this RFP with any State employee other
than the Issuing Officer, except at the direction of the Issuing Officer or as otherwise noted in the RFP. The
Issuing Officer will respond only to questions regarding the procurement process.
2.3 Downloading the RFP from the Internet.
The RFP and any related documents such as amendments or attachments (collectively the “RFP”), and responses
to questions will be posted at the State of Iowa’s website for bid opportunities: http://bidopportunities.iowa.gov/.

version. The Agency will only be bound by the official version of the RFP document(s). Bidders should ensure
that any downloaded documents are in fact the most up to date and are unchanged from the official version.

2.7 Questions, Requests for Clarification, and Suggested Changes.
Bidders may submit written questions, requests for clarifications, and/or suggestions for changes to the
specifications of this RFP (hereafter “Questions”) by the due date and time provided in the Procurement
Timetable. Bidders are not permitted to include assumptions in their Bid Proposals. Instead, bidders shall
address any perceived ambiguity regarding this RFP through the question and answer process. If the Questions
pertain to a specific section of the RFP, the page and section number(s) must be referenced. The Agency prefers
to receive Questions by electronic mail. The bidder may wish to request confirmation of receipt from the Issuing
Officer.
Written responses to Questions will be posted at http://bidopportunities.iowa.gov/ by the date provided in the
RFP’s Procurement Timetable.
The Agency assumes no responsibility for verbal representations made by its officers or employees unless such
representations are confirmed in writing and incorporated into the RFP. In addition, the Agency’s written
responses to Questions will not be considered part of the RFP. If the Agency decides to change the RFP, the
Agency will issue an amendment.
2.8 Submission of Bid Proposal.
The Bid Proposal shall be received by the Issuing Officer by the time and date specified in the Procurement
Timetable. The Agency will not waive this mandatory requirement. Any Bid Proposal received after this
deadline will be rejected and will not be evaluated.
