1. PROJECT OVERVIEW
The City of Lake Ozark is soliciting competitive sealed bids from qualified contractors to provide complete
demolition services for twenty-three (23) structures identified in the City's approved FY25 CDBG Competitive
Demolition Project.
This procurement is conducted in accordance with Missouri CDBG Procurement guidelines (Chapter 9), 2 CFR
200.319, and applicable federal and state requirements. All work will be performed after licensed asbestos
abatement has been completed on the properties that require it by a separate licensed abatement contractor.
The project will eliminate slum and blight conditions in accordance with the City's ordinances and the CDBG
national objective of Elimination of Slum and Blight on a Spot Basis (SBS).
Important Timeline Note: Because asbestos abatement will be completed prior to the start of the demolition
window, the 60-day demolition schedule will be strictly enforced. Requests for additional time beyond
weather-related delays are not anticipated and will not be routinely granted. The process has been streamlined;
bidders should plan accordingly.
2. SCOPE OF WORK - 23 PROPERTIES
The following 23 structures require full demolition. Abatement of regulated friable asbestos has already been
(or will be) performed separately by a licensed abatement contractor. The demolition contractor is responsible
for proper handling of any remaining non-friable ACM using work practices that keep materials non-friable and
in compliance with all applicable regulations.

3. BID DOCUMENTS

4. BID SUBMISSION REQUIREMENTS
Sealed bids must be submitted to the office of the City Clerk at the address above no later than 2:00 p.m. CDT
on SEPTEMBER 10. Bids will be publicly opened and read aloud at that time and place.
* No faxed or emailed bids will be accepted.
* Each bid must be accompanied by a bid guarantee equivalent to five percent (5%) of the total bid
amount. The guarantee may be in the form of a bid bond or certified check and must be included with
the bid.
* No bidder may withdraw its bid within sixty (60) days after the actual date of the bid opening.
* Submit 1 original and 5 copies of the complete bid package in the exact order shown in Section 5.

5. REQUIRED BID SUBMITTALS (1 original + 5 copies)
To be considered responsive and responsible, bidders must submit the following in the exact order listed:
A. Required for Responsiveness
1. Completed and signed Bid Form (total package price in numbers and words)
2. Bid Breakdown by Property - Individual price for each of the 23 structures (the official award will be
based on the total package price)
3. Proposed Demolition Schedule - Detailed schedule covering the full 60-day demolition window,
including the number of days allocated to each property
4. Bid Guarantee (5% bid bond or certified check)
5. Acknowledgment of all Addenda on the Bid Form
6. Bidder's legal name, address, and authorized signature
B. Required to Demonstrate Responsibility
1. Experience statement - similar demolition projects completed in the last 5 years (project name, location,
owner, contract amount, and contact)
2. At least three (3) recent references from past project owners/agencies (business name, address, and
phone number)
3. Proof of current Missouri business registration / Certificate of Good Standing
4. Letter from surety confirming ability to provide 100% Performance and Payment Bonds
5. Certificate of Insurance meeting Contract requirements
6. E-Verify / Work Authorization Affidavit
7. Anti-Lobbying Certification
8. Documentation of OSHA 10-hour construction safety training for supervisory personnel
9. Identification of Disposal Site - Name and location of the landfill/dump to be used for non-asbestos
demolition debris
10. Identification of Fill Dirt Source - Location of fill dirt source (must be acceptable to the State; costs
must be included in the bid)
The City may request additional information after bid opening to verify responsibility. Incomplete bids or bids
missing the required bid guarantee, property breakdown, schedule, or addenda acknowledgment will be rejected
as non-responsive.
6. KEY PROJECT CONDITIONS (READ CAREFULLY)
60-Day Schedule Enforcement Asbestos abatement will be completed before the demolition window begins.
The 60-day demolition period will be strictly enforced. Weather events are the primary recognized exception.
Requests for additional time for other reasons are not expected to be granted.
Change Orders
* Change orders will not be accepted for items that could or should have been materially known at the
time of bidding.
* Only conditions that could not have been reasonably known at the time of bid will be considered for
change orders through the normal process.

Dump & Fill Dirt Bidders must identify both the disposal facility for non-asbestos materials and the source of
fill dirt in their bid package. All related costs must be included in the base bid. The City and State will verify
acceptability of both locations.
Construction / Safety Fencing The City will confirm at the mandatory pre-bid meeting whether safety fencing
will be required prior to or as part of the demolition work. Bidders should be prepared to discuss and, if
required, include related costs.
7. FEDERAL AND STATE REQUIREMENTS
Attention of bidders is particularly called to the following requirements that will be observed under the contract:
* Davis-Bacon and/or applicable Missouri Prevailing Wage rates
* Section 3 requirements
* Segregated Facilities
* Section 109 of the Housing and Community Development Act of 1974; 24 CFR Part 6 and 24 CFR
570.602
* Executive Order 11246 (Equal Employment Opportunity)
* OSHA 10-hour construction safety course compliance
* Documentation of compliance with E-Verify

8. RIGHT TO REJECT
The City of Lake Ozark reserves the right to waive any informality or to reject any or all bids when it is in the
best interest of the City. The contract will be awarded to the lowest responsive and responsible bidder based on
the total package price.
9. QUESTIONS
All questions regarding this Invitation for Bids must be submitted in writing to the City Clerk no later than five
(5) business days prior to the bid opening date. Responses will be issued by addendum to all plan holders.

DEMOLITION SERVICES CONTRACT

THIS DEMOLITION SERVICES CONTRACT (this "Contract") is entered into as of the ______ day of
____________________, 2026 (the "Effective Date"), by and between:
CITY OF LAKE OZARK, MISSOURI, a municipal corporation organized under the laws of the State of
Missouri, with its principal office located at 3162 Bagnell Dam Boulevard, Lake Ozark, Missouri 65049 (the
"City" or "Owner");
and
________________________________, a ________________ organized under the laws of the State of
________________, with its principal place of business at ________________________________ (the
"Contractor").
The City and the Contractor are collectively referred to as the "Parties" and individually as a "Party."
RECITALS
WHEREAS, the City has been awarded Community Development Block Grant (CDBG) funds from the
Missouri Department of Economic Development ("DED") for the demolition of blighted structures;
WHEREAS, the City issued an Invitation for Bids (Competitive Sealed Bids) for demolition services covering
twenty-three (23) properties located within the City of Lake Ozark, Missouri;
WHEREAS, the Contractor submitted a responsive sealed bid dated, [INSERT MONTH AND DAY] 2026,
proposing to perform the demolition services for a total Contract Price of $;[INSERT AMOUNT]
WHEREAS, the City has received Authority to Use Grant Funds (AUGF) from DED and has satisfied all
environmental review requirements for the Project;
WHEREAS, this Contract is funded in whole or in part by federal Community Development Block Grant funds
administered by the U.S. Department of Housing and Urban Development ("HUD") and is subject to federal
requirements including, but not limited to, Davis-Bacon prevailing wage requirements, Section 3 economic
opportunity requirements, Equal Employment Opportunity obligations, and all other applicable federal and state
requirements;
NOW, THEREFORE, in consideration of the mutual covenants, agreements, representations, and warranties set
forth herein and for other good and valuable consideration, the receipt and sufficiency of which are hereby
acknowledged, the Parties agree as follows:
Article 1 - DEFINITIONS
1.1 "Addendum" or "Addenda" means any written or graphic instrument issued by the City prior to the bid
opening that modifies or interprets the Bid Documents.

1.2 "Bid Documents" means the Invitation for Bids, Information for Bidders, Bid Form, Technical
Specifications, Contract Documents, Exhibits, and all Addenda.
1.3 "Change Order" means a written order signed by the City and the Contractor authorizing an addition,
deletion, or revision in the Work, or an adjustment in the Contract Price or Contract Time.
1.4 "City" or "Owner" means the City of Lake Ozark, Missouri.
1.5 "Contract" means this Demolition Services Contract, including all Exhibits, the Bid Documents, Technical
Specifications, Addenda, and any duly executed Change Orders.
1.6 "Contract Price" means the total lump-sum amount set forth in Article 3, as adjusted by Change Order.
1.7 "Contract Time" means the sixty (60) calendar day period allotted for Substantial Completion of the Work,
as adjusted by Change Order.
1.8 "Contractor" means the person or entity identified as such in the opening paragraph of this Contract.
1.9 "Day" or "Days" means calendar days unless otherwise stated.
1.10 "Final Acceptance" means the City's written acceptance of the Work after all punch-list items have been
completed and all required close-out documents have been submitted.
1.11 "Notice to Proceed" means the written notice issued by the City authorizing the Contractor to commence
the Work.
1.12 "Project" means the City of Lake Ozark FY25 CDBG Competitive Demolition Project, Project No. 2024-
DM-10-RC.
1.13 "Site" means the locations of the twenty-three (23) structures listed in Exhibit A.
1.14 "Substantial Completion" means the stage when the Work is sufficiently complete so that the City can
occupy or utilize the Site for its intended purpose, with only minor punch-list items remaining.
1.15 "Work" means the entire demolition and related services required by the Contract Documents, including
all labor, materials, equipment, and services necessary to demolish the structures listed in Exhibit A and
complete site restoration as specified.
Article 2 - SCOPE OF WORK
2.1 The Contractor shall furnish all labor, materials, equipment, tools, transportation, supervision, and services
necessary to completely demolish the twenty-three (23) structures listed in Exhibit A and more particularly
described in the Bid Documents and Technical Specifications.
2.2 Work includes, but is not limited to:
* Complete demolition of all structures, foundations, slabs, footings, and related improvements;
* Proper handling, removal, and disposal of all demolition debris;
* Proper handling of any remaining non-friable asbestos-containing materials;

* Dust suppression, erosion and sediment control, and site stabilization;
* Backfilling, grading, and rough site restoration;
* Compliance with all Environmental Review Conditions (Exhibit D);
* Coordination with the City so that demolition does not begin on any structure until written clearance is
provided confirming asbestos abatement (if required) is complete.
2.3 The Contractor shall use only the disposal facility and fill-dirt source identified in its bid (or a substitute
approved in writing by the City). All related costs are included in the Contract Price.
2.4 The Contractor shall perform the Work in a workmanlike manner in accordance with the Contract
Documents, all applicable codes, ordinances, and regulations, and to the satisfaction of the City.
Article 3 - CONTRACT PRICE AND PAYMENT
3.1 The total Contract Price for the complete performance of the Work is $____________________________
(the "Contract Price").
3.2 The Contract Price is a firm, fixed, lump-sum amount based on the total package price submitted by the
Contractor. The property-by-property breakdown submitted with the bid may be used by the City for progress
payment purposes or partial acceptance but does not alter the lump-sum nature of this Contract.
3.3 Progress payments may be requested monthly based on work satisfactorily completed and accepted by the
City. Each payment request shall be accompanied by certified payrolls, lien waivers, and such other
documentation as the City may require. The City shall retain ten percent (10%) of each progress payment until
Final Acceptance.
3.4 Final payment shall be made within thirty (30) days after Final Acceptance, receipt of all required close-out
documents, and satisfaction of all Contract conditions.
3.5 The City's obligation to make payments is contingent upon the availability of CDBG funds and the
Contractor's compliance with all federal and state requirements.
Article 4 - BONDS AND INSURANCE
4.1 Within ten (10) calendar days after the Effective Date, the Contractor shall furnish:
* A Performance Bond in the amount of one hundred percent (100%) of the Contract Price (Exhibit E);
and
* A Payment Bond in the amount of one hundred percent (100%) of the Contract Price (Exhibit F).
4.2 The Contractor shall maintain the insurance coverages required by the Contract Documents throughout the
term of the Contract. The City shall be named as an additional insured on the General Liability and Automobile
Liability policies. Certificates of insurance shall be provided prior to commencement of Work.

Article 5 - FEDERAL AND STATE REQUIREMENTS
5.1 This Contract is subject to all applicable federal and state requirements, including but not limited to:
* Davis-Bacon Act and related prevailing wage requirements (Exhibit C and HUD Form 4010);
* Section 3 of the Housing and Urban Development Act of 1968;
* Executive Order 11246 (Equal Employment Opportunity);
* Copeland Anti-Kickback Act;
* Contract Work Hours and Safety Standards Act;
* Missouri Prevailing Wage Law (to the extent it requires higher rates);
* E-Verify / work authorization requirements;
* All Environmental Review Conditions set forth in Exhibit D.
5.2 The Contractor shall include all applicable federal and state clauses in every subcontract.
5.3 Weekly certified payrolls (Form WH-347 or equivalent) shall be submitted to the City. Applicable wage
determinations shall be posted at each work site.
Article 6 - TIME FOR PERFORMANCE
6.1 The Contractor shall commence Work within ten (10) calendar days after receipt of the City's written
Notice to Proceed.
6.2 The Contractor shall achieve Substantial Completion of all Work within sixty (60) calendar days after the
Notice to Proceed date (the "Contract Time").
6.3 Because asbestos abatement will be completed prior to the start of the demolition window, the 60-day
Contract Time will be strictly enforced. Weather events are the primary recognized basis for time extensions.
Requests for additional time for other reasons are not anticipated and will not be routinely granted.
6.4 The detailed demolition schedule submitted with the Contractor's bid (showing the number of days
allocated to each property) is incorporated [INSERT CONTRACTOR'S DEMOLITION SCHEDULE] by
reference. The Contractor shall prosecute the Work in general accordance with that schedule unless modified by
written Change Order.
6.5 Time is of the essence. If the Contractor fails to complete the Work within the Contract Time (as adjusted),
the Contractor shall pay the City liquidated damages in the amount of $[INSERT AMOUNT] ______ per
calendar day for each day of delay.
6.6 Extensions of time may be granted only by written Change Order for excusable delays and only if the
Contractor provides written notice within five (5) calendar days after the occurrence of the delay.
