Project Description:
Work to be bid upon as follows: Demolition of building at 104 Indian Center Court,
Kingsport, TN. Work included but not limited to, total demolition of structure, down to slab on
grad, barricades, fencing, erosion control, and other associated work. The allotted time for
construction will be 90 calendar days.

Request for Quotes Concerning Demolition of former Prime Sirloin Building
Please provide a price for the below and attached described work for the demolition of former
Prime Sirloin Restaurant building at 104 Indian Center Court Kingsport, TN. Work includes
demolition of building down to slab on grade to remain. Existing asphalt paving to remain.
1. Pre-Bid Meeting on site Monday March 16th at 10:00 a.m.
2. Contractor must obtain a Demolition Permit from the City of Kingsport Building Department.
3. Notification of Demolition and/or Asbestos Renovation must be submitted to TDEC on CN-1055
Notification Form, 10 working days prior to demolition as per State regulations.
4. City of Kingsport Building Department will not issue demolition permit until permit with State has
been filed.
5. Tennessee One call www.tenn811.com by calling TN811 must be done at least 3 working days prior
to any excavating work if warranted on project.
6. The existing AEP transformer is connected to other adjacent properties and is to remain. The
transformer must be protected during demolition operations.
7. AEP representative Tom Hensley must be notified ten days prior to building demolition. Contact
information for Tom Hensley is office # 423-578-2240 and email address tdhensley@aep.com
8. A Pre-Demolition Hazardous Material Assessment Report has been completed and prepared by
Mattern & Craig dated October 31, 2025, and have been made a part of the bid documents.

9. Proper notice must be filed with Tennessee Department of Environment & Conservation Division of
Air Pollution Control, Asbestos NESHAP Program.
10. Demolition Contractor must be properly licensed and insured by the State of Tennessee and
comply with all applicable Federal, State and local regulations.
11. Property must be demolished in a manner that is safe to the adjoining and surrounding businesses
and residences.
12. Contractor shall be responsible for maintaining a safe working environment for workers and
citizens.
13. Be considerate of the adjacent property owners’ yards, landscaping, etc. Take pre-construction
videos and pictures.
14. Contractor to demolish building structure down to existing slab on grade. Slab on grade, asphalt
parking and parking lot lights to remain.
15. There shall be no fires of any kind or burning of any debris.
16. All materials trash, garbage, refuse, brush, debris and building materials must be removed and
disposed of at an approved landfill appropriate for the type of material being disposed of.
17. Contractor must keep the surface of the sidewalks and streets affected by their work in a clean and
safe condition.
18. Contractor does not need to remove trees larger than 3” in diameter, if applicable.
19. The grade must be restored to adjacent established grades, unless noted otherwise per plans for
slabs to remain. All grass, weeds and overgrowth will be cut to within 3” of the ground. All
disturbed property to be seeded and strawed as per City of Kingsport Specs.
20. Contractor shall be responsible for all traffic control per City of Kingsport specs, Erosion and
Sediment Control measures as per TDEC (Tennessee Department of Environment & Conservation)
Erosion Prevention & Sediment Control (EPSC) Handbook, latest edition.
21. Utilities must be terminated and property capped at the property line.
22. A final demolition inspection is required with the City of Kingsport Building Department. Call
Building Department at 423-224-2717 to make appointments.
23. Pay application request to be submitted on AIA Document G702 and Continuation Form G703
latest version or digital version printed with three (3) original signed copies. If contractor does not

have a subscription to AIA documents, then payment may be submitted with similar format
consistent with AIA.
24. Each Pay request must include three (3) original signed copies of Certificate of the Contractor (see
attachment).
25. Final Pay application must include Certificate of the Contractor Form, original signed & notarized
Waiver of Lien (see attachment), and Consent of Surety (if applicable if Bid Bond is required).
