STATEMENT OF WORK

GENERAL INFORMATION

Title of Project: Olympus Endoscopy Support

Scope of Work: The vendor will provide support for the Olympus endoscopy equipment, listed below, that is currently in use at the Baltimore VA Medical Center. Support will include:
Full repair coverage with no cap for all contracted equipment, including accidental damage.
Next day shipping in both directions included at no extra charge
Prioritized repairs, with 24 - 48-hour turnaround
Access to loaner equipment at no extra charge
24/7 technical support
Online repair tracking and history
Periodic and on-demand training and support provided by trained support specialists
Access to preventative maintenance and educational courses
Service Contracts administrative support during typical business hours

All repairs must return the equipment to the device s original or current manufacturer specifications. The vendor will supply the Baltimore VAMC Biomedical Engineering department with a copy of a detailed service report that meets the requirements of 21CFR820, subpart N, within 2 weeks of a repair being completed. All exchanges must be documented with an RMA provided to the Baltimore VAMC Biomedical Engineering department including the serial numbers of both the equipment being replaced and the replacement equipment.

Background: The VA Maryland Health Care System requires full-service coverage of the Olympus endoscopy equipment at the Baltimore VA Medical Center. This service coverage includes unlimited repairs and exchanges for the covered products that return the equipment to the original specification to ensure the safety and performance of the covered equipment as well as the efficacy of the documented reprocessing procedures.

Performance Period: 1 base year plus two option years
