This Request for Information (RFI) is issued in accordance with FAR 10.001 to determine the availability and technical capabilities of qualified sources. Please respond to the questions in section 3.0.
The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, Surgical Appliance and Supplies Manufacturing, with a small business size standard of 800 employees. The FAR Provision found at 52.204-7(b)(1) requires that all offerors obtain a UEI to register in SAM. You may access the SAM website at https://www.sam.gov.

2.0 DESCRIPTION OF ANTICIPATED NEED
VHAWNYHS Buffalo Surgical Department requires a procurement of CABG Suture Packs designed for open heart cardiac surgeries performed in the operating room. The CABG Suture Packs are used for Cardiac surgeries only and help to reduce time and inventory of individual sutures required to keep on hand. Due to the CABG Suture Packs being service specific, we maintain inventory and we purchase when the stock becomes low. We intend to purchase three (3) cases. Each case contains:

All items must be delivered to the Buffalo VA Medical Center at 3495 Bailey Ave, Buffalo, NY 14215.
