**Must be a Medicaid enrolled Environmental Modification service provider (ODD/DDS) to bid. Provider requirements packet will be given upon request** Widen exterior door to a minimum of 36 inches, widen three interior doorways (bedroom, bathroom, closet) to accommodate wheelchair access. Include all finishing work (trim, drywall, paint, vinyl flooring where necessary).
