Self-Direction Fiscal Intermediary and Support Brokerage
TO ALL PROSPECTIVE BIDDERS:
The following questions were submitted by a potential bidder. Our answers are
listed below in red:
1. Will bids be accepted from an agency with a minimum of 5 years’ experience providing
goods & or services?
Must attest to a minimum of 5 years’ experience providing the good and/or services described
in this bid document.
2. Under Section II Minimum Proposer Requirements number 6, can two or more agencies
collaborate?
Shelby County will contract with one vendor. All subcontract relationships must be approved
prior to implementation by the contractor. The entity contracted with must have a minimum
of 5 years’ experience.
3. Can you please confirm the current number of participants and how many participants you
anticipate over the life of this contract? How many referrals of new participants for each
program are sent each month? How many participant terminations for each program occur
each month? What has been the growth rate of the program over the last 3-5 years?
None at this time. Persons who are eligible choose these services. We are unable to predict
how many the department will choose.
4. How many caregivers/employees are directly hired by the participant? How many providers
were issued a W2 for 2025?
Only 1 caregiver received 2025 W2.
5. Does the program require face-to-face enrollment visits or are virtual and/or phone
enrollments allowed? What is the current timeframe for referral to start of services for a
newly referred participant and for a directly hired caregiver/employee?
Please include how your organization plans to conduct this part of service in proposal’s detail
narrative (program description, policies, etc.)
6. How often is the support broker required to make contact with the participant? Is there an in-
person requirement?

narrative (program description, policies, etc.)
7. What is the average participant budget size, as well as the minimum and maximum budget
sizes? Is the participant allowed to purchase goods and services, or are they restricted to only
hiring caregivers/employees?
Budget is capped at $7850 per person, the broker will manage caregiver services. Other parts
of budget can purchase other services outside of caregiver.
8. Does the AAAD pre-fund services? If not, what is the timeframe for reimbursing for
provider-related claims, including the financial management PM claim? What is the
format for how claims are submitted?
No prepayment, this is cost reimbursement. Generally, payment is 45 days after invoice is
submitted. Invoice is due 5th of month following service.
9. What is the current employer payroll tax, including FICA, FUTA, SUTA, and workers
compensation insurance? In other words, if a provider was paid $10 per hour, what
hourly claim rate would the AAAD be billed by the FMS?
The Fiscal Intermediary negotiates the rate paid to caregiver and pays associated employment
taxes and fees.
10. Who is the incumbent provider?
None.
11. What is the current PMPM for FMS and Support Broker Services? Are there fees for
onboarding/training new participants and new caregivers/employees?
As part of the usual and customary rates requirement of the RFP, these should be proposed.
12. Is Electronic Visit Verification (EVV) a requirement for the contracted FMS? If so, what
specific guidelines must be followed and who is the aggregator?
Not required, if you do this, please include how your organization plans to conduct this part
of service in proposal’s detail narrative (program description, policies, etc.)
