CLYDE LASSEN SVNH SIX (6) PATIOS ROOF REPLACEMENTS

FDVA invites interested Contractors to submit a bid in accordance with this solicitation. The purpose of this solicitation is to establish an Agreement with FDVA for the Clyde E. Lassen State Veterans’ Nursing Home located at 4650 State Road 16, St. Augustine, FL 32092 (St. Johns County). The successful Contractor shall provide all labor, services, equipment, tools, materials, parts, and supplies required in the safe performance of the Agreement. Contractor shall successfully remove and replace the six (6) patio roofs, all the associated and damaged underlayment, insulation and interior drywall ceilings including off-site proper disposal of all the project related waste. For further specific details, see Section III “Statement of Work”. The term of this Contract shall commence on the date of the Agreement’s full execution, with no renewals. FDVA requires specified services to be completed to the full satisfaction and acceptance of FDVA and any applicable authorities having jurisdiction, within ninety (90) calendar days from the date of the Agreement’s full execution.
