QA Set 1

Q1. Is there any incumbent/recompete or this is a brand-new project? If yes, could you please provide us with their award number or even if the customer bought similar services in the past.
A1. This is a new contract. There is no previous contract for this requirement.

Q2. Instructions at the top of the fill in portion on page 1 of the application suggest to fill out items, 12,17,23,24,30. On the specific instructions (section E) and also aligning with the highlighted portions - only 12, 17, and 30 are required. Assumption because item B is a culmination of what would be in those missing fields.
A2. Please reference E.1 Instructions to Offerors B. Specific Instructions regarding the SF1449.

Q3. Past performance questionnaire:
What is Company/Division name ? Is that the name of my company (the offeror)
A3.1 Yes, this is the Offeror. The Evaluator is to complete their information in the signature block at the bottom.

If my clients/customers are supposed to email you them directly, is there anything that is on this document that will link their performance review to mine?
A3.2 Yes, A. Reference History references the Offeror s History & Contact Information.

How do I indicate that the work was performed as prime - we don t have a written contract but we do give them a price sheet and limits of liability if they choose to use our services.
A3.3 If you do not have a contract but more of a Memo of Understanding with a price sheet, your Past Performance can still be used. Please explain in the question boxes below or where applicable.

The past performance questionnaire is 3 pages already, are we supposed to use that as our template or just ensure all of the key points are covered.
A3.4 E.1 Instructions to Offerors B, 2. Part II Past Performance Information states 4 pages.

Is the quality and satisfaction rating supposed to be attached to the questionnaire?
A3.5 Yes, this information can be attached to the questionnaire or attached to your quotation package and reference the expected Past Performance submitting.

Is the BAA form online the correct document (it states Philadelphia which was my concern)
A3.6 Attached is an updated BAA. Please use this document for your RFQ submission.
