wooden coffee table, approx. 48” long.

Qty 2 - (FIRST Floor #4) swivel lounge chairs and 2-seat midback sofa (no arms) with low, long
wooden coffee table, approx. 48” long.

Qty 1 - (FIRST Floor #5)seat sofa bench (no back, no arms) by itself, 52” wide or less.

Qty 1 - (FIRST Floor #6)Rolling Cart for microwave/food prep, approx. 30” wide.

Qty 1 - (SECOND Floor #1) Rectangular wood or laminate top dining table (approx. 60 - 72”
long), with 4 contoured dining chairs.

Qty 1 - (SECOND Floor #3) Rectangular wood or laminate top dining table (approx. 60 - 72”
long), with 4-5 contoured dining chairs.

Qty 1 - (SECOND Floor #2) Rectangular wood or laminate top dining table (approx. 60 - 72”
long), with 2 booth-style padded midback benches (no arms, approx. 60 -72” long).

Qty 1 - (SECOND Floor #4) 1-seat midback sofa bench (no arms), 56” wide or less, with 2 low
wooden side tables, approx. 20-24” across.

Qty 1 - (SECOND Floor #5) Round wood/laminate table, approx. 36” across, with 4 round, ottoman-
style stools.

Qty 1 - (SECOND Floor #6)Rolling Cart for microwave/food prep, approx. 30” wide.

Special Instructions

Under HRS 103D-310(c) Vendors must provide proof of compliance with all laws governing
entities doing business in the State of Hawaii via Hawaii Compliance Express - To
register
go to vendors.ehawaii.gov/hce. Vendors must attach Certificate of Compliance to their
bid
listing them as COMPLIANT through the issuance of the purchase order or bid will be
void.

1) All substitutions must be approved in advance (2 days before closing date). We
reserve the right to determine equivalency. Accepted equivalent items will be shared as
an addendum to this request.

2) Bidders must provide quotations on ALL items to be considered for award. Award will
go to lowest responsive and responsible offeror on total aggregate quotation.

3) Please include product specifications and supporting documentation with your quote
to verify compliance with the requested requirements.

4) Quotations shall include all charges and applicable taxes, i.e. material/non-
material
costs, foreign taxes, transportation, vehicle costs, air freight, shipping, supply
costs,
mobilization costs, insurance premiums, warranty costs, overhead costs, expediting
fees,
taxes (Federal and State).

5) Quantity subject to change based on funding. Quote must be valid for 90 days.
Delivery 90 days After Receipt of Order (ARO).

6) Winning bidder must return signed award acknowledgment form within 3 working days of
being
awarded.

7) The submittal of a bid shall be considered as acceptance of the specifications as
published as well as agreement to the University of Hawaii Terms and Conditions,
Applicable to All Purchase Orders. Please see link to UH Purchase Order Terms and

8) Vendors need to obtain a copy of their
\"Certificate of Vendor Compliance\" via Hawaii Compliance Express and attach it to their
bid. To register with Hawaii Compliance Express, go to:

9) Vendor (preferably based on the island of Oahu in the state of Hawaii) must be able
to provide service and support within 48 hours of request for warranty service or
repair. Vendor must be authorized dealer for furniture. Direct from the manufacturer
only. Once awarded, a Certificate of Liability Insurance is required.

Mandatory Site Visit and Verification of Conditions:

All prospective bidders shall conduct a site visit at the project location prior to the
submission of a bid. The site visit is required for the purpose of verifying existing
conditions and obtaining accurate measurements necessary to determine the type,
dimensions, and configuration of furniture that may be furnished under this
solicitation.

The bidder
shall be solely responsible for ensuring that all proposed furnishings are compatible
with the actual site conditions. Failure to conduct a site visit may result in the bid
being determined non-responsive. (All Interested vendors please contact Bryce Myers at
