HAZARDOUS WASTE REMOVAL - FM25-26

Offerors must submit a proposal by email and be received no later than 3:00 pm on Monday, March 17, 2025. Any proposal received after this time and date will not be accepted.

The proposal package must be submitted according to Section III, “Bid Submission Packet”, and must include:
1. The Invitation for Bid form, completed according to Section XII, “Pricing”,
2. Signed “Bid/Proposal Affidavit Form” (Attachment 1),
3. Signed “Conflict of Interest Affidavit and Disclosure Form” (Attachment 2),
4. Current Maryland licenses or certifications, per section II.D [“Permits, Certificates, Laws...”],
5. Current insurance certificates, per section II.C [“Permits, Certificates, Laws...”],
6. Completed Reference Form (Attachment 6),
7. Required evidence of specified required safety trainings and insurance as defined in Section IV, “Safety”,
8. Completed No Reply Form if not bidding (Attachment 5).
9. Acknowledgement of receipt of any addendums to this IFB.
