Summary
  1. MISSION & GUIDING PRINCIPLES
The mission of the Ohio Department of Mental Health and Addiction Services (OhioMHAS) is to provide statewide leadership of a high-quality mental health and addiction prevention, treatment and recovery system that is effective and valued by all Ohioans. OhioMHAS strives to be a national leader in implementing a comprehensive, accessible, and quality-focused system of addiction and mental health care and wellness for all Ohio citizens.
  1. PURPOSE
The Ohio Department of Mental Health and Addiction Services ( Ohio MHAS) is seeking proposals from qualified contractors to provide on-site dental services at our Northwest Ohio Behavioral Healthcare, Northcoast Behavioral Healthcare and Heartland Behavioral Healthcare facilities.
  1. BACKGROUND
All services will be provided on the hospital(s) grounds for our mentally ill patients .
  1. SCOPE OF WORK AND DELIVERABLES
Northwest Ohio Behavioral Healthcare (NOBH) is located at:

Toledo, Ohio 43614

Heartland Behavioral Healthcare (HBH) is located at:
3000 Erie Street South
Massillon, Ohio 44646

Northcoast Behavioral Healthcare (NBH) is located at:
1756 Sagamore Road
Northfield, Ohio 44019

The vendor shall provide all necessary dental equipment, instruments, and dental supplies for each visit. The vendor will certify and provide documentation that all appropriate staff is duly licensed dentists or dental hygienists through the Ohio State Dental Board and remain as such during the duration of this contract.

Each location will provide the contractor will adequate, cleared and sanitary space to set up dental equipment in order to provide treatment. Adequate space shall include access to water and electricity.

The NOBH Director of Nursing will monitor the clinical performance of the quality of dental services provided on a quarterly basis and collaborate with the Chief Clinical Officer. The HBH Chief Nursing Officer will monitor the clinical performance of the quality of dental services provided every 2 months and collaborate with the Chief Nursing Officer. The Northcoast QA/PI department will monitor the clinical performance of the quality of dental services provided on a quarterly basis and collaborate with the Office of the Chief Clinical Officer. The vendor will be responsible for continuously monitoring the quality of services and outcomes of services provide. Such as findings will be reported and communicated to the facility contact person on a regularly established basis.

All medical equipment will meet the requirements for all Joint Commission and CMS Hospital Accreditation Standards. All radiology equipment, operators and facilities will be the standards and licensure as prescribed by the Ohio Department of Health & Addiction Services. A licensed biomedical equipment technician or testing firm will inspect, test, and attach appropriate labeling to all medical equipment before it is placed in our service at Northcoast Behavioral Healthcare grounds and thereafter on a semi-annual basis each calendar year. All reusable dental instruments brought into each location will have accompanying sterilization cycle and spore testing documentation showing successful sterilization.

Dental Services shall include:
•Routine examinations
•X-Rays
•Prophylaxis (cleaning)
•Amalgam and composite restorations (filings)
•Simple extractions
•Complete and partial dentures, including necessary repairs and relining
•Other necessary services within the scope of the staff dentist*

*Based upon the acute and immediate nature of the dental needs and the physical and mental condition of the patient.

** Solicitation can be awarded as a multiple award contract, per location, if it is advantageous for the department.
  1. MINIMUM QUALIFICATIONS OF CONTRACTOR

The supplier will certify and provide documentation that all appropriate staff is duly licensed dentists or dental hygienists through the State of Ohio Dental Board and remain as such during the duration of the contract.

Contractor shall not be subject to an “unresolved” finding for recovery under Section 9.24 of Ohio Revised Code.

Process

MHAS’s evaluation process of responses submitted to this request may consist of up to four
distinct phases:
1. MHAS’s initial review of all proposals for timely submission;
2. An evaluation committee review of the proposals for defects and scoring;
3. MHAS’s request for more information (clarifications, interviews, presentations, and/or
demonstrations); and,
4. Negotiations or best offer requests.
At its sole discretion, MHAS will determine whether phases three and/or four are necessary under
this RFP, reserving for itself the ability to eliminate or add phases three or four at any time during
the evaluation process. MHAS may add or remove sub-phases to phases 2 through 4 at any time
if MHAS believes doing so will improve the evaluation process.
