Request for Proposal for San Miguel County Sheriff's Office Jail Based Behavioral Services

It is the intent of this Request for Proposal to obtain proposals from qualified vendors to provide jail-based behavioral services (“JBBS”) to the inmates of the San Miguel County Jail. These services must include:

1. Licensed mental health clinician(s): In-person (primary) and telehealth (secondary)
2. Case Management: Incarcerated, Post incarceration, Recovery Support Services
3. Licensed Physician/Assistant, Nurse Practitioner, Prescription control and issuance
4. Continuity of care and cooperation with health care provider.

Any item not specifically mentioned but necessary for the delivery and operation of the proposed service shall be included in this proposal. These specifications and requirements should be in sufficient detail to secure proposals on comparable services.

San Miguel County serves approximately 8,000 citizens and encompasses a diverse region ranging from the rugged mountain resort communities of Telluride and Mountain Village to the ranching communities of the County’s west end, Norwood and Egnar. The County is approximately 1,288 square miles, stretching from the San Juan Mountains in the east to the Utah border to the west. The incorporated towns in the County consist of Telluride (County seat), Mountain Village, Ophir, Sawpit, and Norwood. Other communities include Placerville and Egnar.

The County Jail is located in the Ilium Valley at 684 CR 63L, Telluride, Colorado 81435. The jail is a 32-bed facility and the average daily population in 2025 was three (3).

1. Due to the complex nature of mental health services within correctional facilities, Vendors must be well experienced in providing this type of service. The Vendor shall provide the properly licensed staffing and have the capability to utilize the State tracking system called, “Civicore.”
2. The Vendor shall be able to provide the required services primarily face-to-face (and via telehealth if necessary).
3. The Vendor shall provide therapy/case management for each inmate enrolled in the program, keep the documentation updated in the Civicore system, and any other documentation required by the grant through the State Office of Behavioral Health.
4. The Vendor must have the availability to work through an established schedule that accommodates the open times within the jail facility.
5. The Vendor’s employees are subject to CJIS Security training and requirements (e.g. fingerprint-based background check) and may not be allowed or accepted into the San Miguel County Jail for any reason at any time.

1. The reports to the County will contain a variety of information to suit the County’s needs and the needs of the grant that provides the funding for this service.
2. The service provider must be capable of providing web accessible real-time and historical reports at the request of the San Miguel County Jail in order to meet grant requirements.

1. Vendor may provide mental health first aid and other related training at the cost of the grant funds if requested.
2. The vendor must have the ability to provide initial and ongoing training through multiple options such as live training and virtual.

1. Vendor shall be required to report client information into databases selected by the Behavioral Health Administration. For SFY26, BHA has elected to use Civicore, owned and operated by “NeonOne”, and Chestnut Health Systems, Inc. (“Chestnut”), the sole provider of the GAIN (Global Appraisal of Individual Needs). Each agency's user agreements must be in place with BHA and, or each individual jail. BHA reserves the right to change the client information databases at its discretion. Any such change will be announced ahead of time and shall provide further instructions regarding usage and user agreements.
2. Data must reflect current enrollment of all program participants, along with the services provided, by the 15th day of each calendar month to allow BHA staff to utilize current data. The following data elements shall be captured in the Civicore JBBS database, or another database as prescribed by BHA: A record for each individual who screened “positive” for a mental health disorder or substance use disorder; other screenings completed (including the GAIN 3.2 when required) and results thereof. Basic demographic and working diagnosis information (including veteran status and pregnancy status, if applicable). The type and dosage of medications provided for Medication Assisted Treatment (MAT). Please see Exhibit B for allowable medications. Number of individuals who successfully transition to community-based services upon release. Program discharge outcomes and treatment status in the community after discharge.
3. The Contractor shall respond to BHA’s inquiries about data submissions within two (2) business days and work with BHA to quickly resolve any data issues.
4. The Contractor shall notify BHA of any staffing changes within 48 hours, as leaving an individual's database access shall be deactivated.
