Bid Type: Request for Proposal Locations: Keywords: Social Services, Developmental Disabilities Bid Description: The State of Vermont, Developmental Disabilities Division (DDSD) and Adult Services Division (ASD), are divisions within the Department of Disabilities, Aging and Independent Living (DAIL) in the Agency of Human Services (AHS), jointly hereafter “State.” ASD is responsible for a full array of long-term services and supports for older Vermonters and adults with physical disabilities. DDSD is responsible for all community-based long term services for individuals with developmental disabilities. The purpose of this RFP is to solicit proposals for one or more Contractors to provide case management services for individuals with Developmental Disabilities and Brain Injuries. A complete description of the Divisions’ developmental disability and brain injury programs can be found at the following websites: Home Page | Developmental Disabilities Services Division (vermont.gov) and Traumatic Brain Injury Program | Adult Services Division (vermont.gov). The State of Vermont is interested in obtaining bids to meet the following business need(s): Vermont is pursuing statewide contracts with one or more organizations to deliver case management for the DS and BIP systems. The State hopes to award multiple statewide contracts depending upon the number of qualified respondents. Starting approximately April 1, 2025, the chosen Case Management Entities (CME) will begin the transition to delivering case management for participants in the DS and BIP Systems.
