CITY OF BROOKINGS TREE REMOVAL SERVICES
TO: THE CITY OF BROOKINGS, SOUTH DAKOTA
The undersigned offeror, having examined the Specifications and other contract documents and
addenda attached hereto and being acquainted with and fully understanding the requirements,
provisions, and conditions of this Project, proposes and agrees that Offeror will enter into and
perform a contract consistent with the attached specifications for the hourly price submitted on
this Proposal Form, and will do the work and furnish the equipment, transportation, and services
for said work in strict accordance with the Specifications, which form a part of the contract
documents; for the price shown on the following proposal items:
One contract will be awarded. The owner reserves the right to reject any and all proposals and to
waive any irregularities in the proposals received. All Offerors are required to have a South
Dakota Sales Tax Permit Number and must provide that number in the proposal.

The above proposal must include State Sales Tax or Excise Tax.
The undersigned, by their signature below, acknowledges that Offeror is familiar with the
Specifications and all Contract Documents related hereto.
The undersigned further agrees that if awarded the Contract for the work described in this
Proposal, will complete and perform the work contemplated in accordance with the conditions
set forth in these Contract Documents.
The undersigned has checked carefully their proposal, and understands that the City of
Brookings, South Dakota, will not be responsible for any errors or omissions on the part of the
undersigned Offeror in making this proposal.
