Central Health is committed toimproving patient health outcomes by addressing food-related issues thatsignificantly impact the well-being of our diverse patient population. ThisRequest for Proposals (RFP) seeks qualified vendors to provide solutions thataddress three primary areas of need: (1) a medically tailored meal deliveryservice providing meals tailored for disease management to patients, (2) theimplementation of mobile food pantries (a “food farmacy”) within CentralHealth’s service areas, and (3) a homelessness-focused food services andemployment program designed to meet the nutritional needs of individualsexperiencing homelessness. The goal of this RFP is to enhance culturallycompetent food access and nutrition support throughout Travis County, therebyimproving patient health and reducing barriers to care.
