NON-PERSONAL SERVICE CONTRACT FOR OPERATING ROOM ASSISTANT (SURGICAL SCRUB TECHNICIAN)

The Indian Health Service, Shiprock Service Unit, Northern Navajo Medical Center, located at US Hwy 491 North, Shiprock, New Mexico 87420 has a requirement to four (4) Operating Room Assistants (Scrub Technicians). These positions will performed in accordance to the attached Statement of work. The Period of Performance will have a base period for 6 months with an option to extend and additional 6 months.

Base: 01/13/2025 to 06/12/2025, Option to extend: 06/13/2025 to 01/12/2026

The North American Industry Classification System or NAICS code is 561320 – Temporary Help Services with a small business size standard of $41.5M

The Request for Quotations No. IHS1503520_3 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be used. Evaluation factors may include technical capability to meet the government need outlined in the statement of work, past performance, and price.

Quotes will be due on December 30, 2024 no later. The acquisition is a set aside as Indian-Owned Small Business.

In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
