STATEMENT OF WORK (SOW)
1. Contract Title. Building 26 Serenity Square Furniture Contract 2025.
2. Background.
Building 26 (Mental Health CLC Patient Care Unit) we moved into the unit with minimal furniture, and we are now trying to finish furnishing the 10-bed unit.

3. Scope.

Equal product can be submitted. Please include all specifications of equal product being submitted. Please submit sample of equal product for interior designer to review.

4. Specific Tasks.

Vendor shall deliver furniture and install the following (Brand or equal product) for Building 26 first floor at the Chillicothe VA Medical Center 17237 State Route 104 Chillicothe, OH 45601.



Deliverables:

Room 175 Dining: QT0Y.10 Ryno Dining Chairs for Dining Room with seat pads. The seat pads are approximately 1.18 inches thick. Seat pads can be replaced if needed.

Specifications for Tables:

Column uprights and feet are constructed of 14-gauge seam welded steel tube with the column being 3 O.D and the base steel disc covering being 16 gauge ranging in diameter to match the top size. An inner 3/8 thick counterweight is inserted under the whole circumference of the disc cover plate for both support and to provide weight to give stability to the top. A ¼ thick steel mounting plate is welded to the top of the column and secured to the top using 3/4 tamperproof bolts. The counterweight is mechanically fastened to the column. Steel shot is added to the column for weight. All fasteners are tamperproof.

Laminate- Nu-Green 2 ULEF ( ultra-low emission formaldehyde) raw particleboard core. The core is made using 100% pre-consumer recycled or recovered wood fiber, and is manufactured inside a FCS Certified manufacturing facility. The top desnitiy is 39 pounds per cu. ft. All connections are metal to meal.

Installation:
Vendor to install all furniture at Building 26 first floor. When install has been completed final walk through required and list of furniture placed into each room required for final sign off on contract.

The Contract Specialist for this request is VA Interior Designer /COR will be the point of contact for this contract.

The contract will not have access to medical or patient information. The C&A requirements do not apply and a Security Accreditation Package is not required.

The furniture shall be brought with the installers to the facility the day of installation. Items shall not directly ship to the VA facility. Truck will need lift gate. Installation is located on first floor of Building 26. There are elevators available. Must take elevators from ground floor to first floor.

e. Inspection and Acceptance Criteria.
The COR is responsible for certifying that the work done under the contract is performed to time and standard. They are also responsible to assure the inspection and acceptance of products provided incidental to services. A demo period or trial is not inspection of a product.
