submissions, or electronic media such as flash drives or CD.
Submissions received after the following deadlines may not be accepted by the City.
Providers with existing contracts – submission due May 6, 2026 by 3:00 PM
New providers – submissions due March 15, 2026 by 3:00 PM
Questions concerning this RFQ must be in writing (email is preferred) and addressed to
the Contract Officer listed on the front page of this solicitation, and, should be received
no later than five (5) business days preceding the date that the qualifications are to be
received. E-mail is the preferred method and will get a more timely response.
Providers who respond to this Request for Qualifications are not guaranteed a contract
by the City, or if offered a contract, that any work will be provided.

1.0 PURPOSE
The intent of this Request for Qualifications is to obtain qualified vendors to provide a wide
variety of services to Suffolk Social Services in compliance with the Children’s Services Act
(CSA). Providers are to submit a Statement of Qualifications (SOQ) in accordance with the
terms and conditions indicated below.
2.0 BACKGROUND
The Children’s Services Act is now the name of the Comprehensive Services Act for At-
Risk Youth and Families (CSA), passed in 1992 and implemented in 1993, to provide for
an innovative Systems of Care approach for Virginia’s children and families. The City of
Suffolk Family Assessment and Planning Team (FAPT) is responsible for identifying the
vendors to provide services for individual cases.
In addition, the Child Abuse Prevention and Treatment Act (42 USC Ch. 67) (CAPTA)
enacted in 1974, reauthorized in 2010 and amended in 2015, 2016, and 2018 provides
federal funding and guidance related to child abuse neglect and passed in and
implemented. The Families First Prevention Services Act (FFPSA), enacted in 2018,
provides additional guidance and funding related to preventing child abuse and neglect
as well as unnecessary placement into foster care. Suffolk Social Services is responsible
for identifying and connecting children and families with these services.
3.0 TERM OF CONTRACT
The contract shall cover the period beginning July 1, 2026 (or upon receipt of signed
Contract) and continuing through June 30, 2028. This is an “as-needed” agreement. It is
understood and agreed that the City shall not guarantee any minimum amount of work. Any
contract that may result from this solicitation shall not obligate the City to utilize the provider
for the services described.
4.0 SCOPE OF SERVICES
The City is looking for vendors who can provide the following services:
Residential (congregate care) programs, Medicaid Level B & C
Private day schools and residential schools for students with disabilities
Shelter care
Treatment Foster Care
Independent living services
Transportation
Community based services (may include any of the following and other services
as received in a community setting: Evidence-Based Programs for children and
families, Applied Behavior Analysis, Assessment/Evaluation, family support
services, individualized support services, mentoring, respite, mental health
services, substance abuse services, sex offender treatment services)
Other additional services as required

All providers who are funded using Children’s Services Act funding are required to attend
FAPT meetings, provide a written treatment plans that correlate with the Individualized
Family Service Plan (IFSP) within 30 days of initiation of services, and provide monthly
progress summaries. When providers are funded utilizing other funding streams,
provider representatives are required to participate in Treatment Team, Child and Family
Team Meetings and/or Family Partnership Meetings as they apply to the families and
children being served. Providers must be cooperative with utilization review procedures
conducted by the CSA staff and/or Social Services staff, and FAPT including allowing
chart and on-site reviews and visitation with the child and staff providing services.
Providers must actively participate with Suffolk Social Services and CSA staff related to
children and families.
For services whose pay rates are approved at the state or federal level, providers must
agree to accept the standardized rate as established by the Office for Children’s
Services, the Virginia Department of Social Services or Medicaid.
Vendors are also highly encouraged to participate in the annual Southeastern Region
CSA Vendor Fair in order to meet agency staff and discuss their services. Vendors may
bring marketing materials during presentations but are not allowed to distribute items that
could be construed as gifts.
5.0 QUALIFICATIONS
Providers must demonstrate that they have the resources and capability to provide the
services as described herein. All providers must submit documentation as indicated
below with their qualifications. Providers must be fully licensed and insured in the
Commonwealth of Virginia. All providers who work directly with children and families
are expected to ensure that employees have been background screened to include, at a
minimum, a clear Child Protective Services check and criminal background check in
accordance with applicable law.
A. Whether the provider is enrolled to provide services for Medicaid reimbursement
B. Current licensure/certification with the appropriate organizations
C. Ability, capacity and skill of the provider to provide the services required,
including verifiable competencies and accreditation
D. Rates for services to be delivered
E. Providers are preferred to possess a clinical license for at least one service or
have supervision provided by a licensed person on staff.
F. Insurance as defined by the RFQ per City of Suffolk requirements
G. Reference letters (at least two from other professionals)
H. Performance data for prior fiscal year. If outcome data is not available, submit
plan for collection of such data for the next fiscal year.
