Please quote the following services, as per the specifications noted below, for furnishing to the Department of Public Service of the City of Wadsworth for the period April 1, 2026, through March 31, 2027. The following conditions apply to the quote and/or to any vendor:

  • pricing to be honored for the period April 1, 2026, through March 31, 2027 (vendor must note any pricing that is for less than that one year period); vendor is responsible for notifying the City of Wadsworth of any change in pricing
  • all prices should be quoted as services performed within the City of Wadsworth, and must include any and all additional costs for fuel surcharges, freight, travel or other additional fees required for delivery of said services.
  • quantities shown are approximate only; actual quantities will depend on the requirements of the Department of Public Service
  • payment will be made for items as outlined below, and per unit items will be paid per unit price as installed and accepted by the City; vendor is aware and agrees that payment for any services may take up to 45 days

SPECIFICATIONS FOR STREET and WATER DEPARTMENT SERVICES
SERVICES
All providers of services shall provide documentation of appropriate insurance coverage, as well
as of income tax withholdings to RITA (Regional Income Tax Agency) Division of Taxation for the
City of Wadsworth. Bidders shall provide contact information for any subcontractors used.
S-1 CONCRETE REPLACEMENT
The contractor shall provide all labor and equipment to remove and replace existing concrete pavement,
aprons, sidewalks and handicap accessible ramps as designated throughout the City in accordance with
the current Ohio Department of Transportation Construction and Material Specifications, 452 and 608.
THE CONCRETE, AGGREGATE BASE, #5 EPOXY COATED REINFORCING STEEL, AND
TRUNCATED DOMES WILL BE PURCHASED AND SUPPLIED BY THE CONTRACTOR. The price bid
shall include all materials, labor and equipment necessary to complete the work.
Labor and equipment shall include, but not be limited to: setting up traffic control, saw cutting existing
concrete pavement, removal of existing pavement, excavation of up to 3” of unsuitable subgrade,
removing spalled joints after removal, hauling and dumping of excavated material, placement of up to 3”
of ODOT 304 Aggregate Base, compaction of subgrade, forming, placement of 18 inch length of #5 epoxy
coated reinforcing steel @ 18” spacing, placement of expansion material, placement of ODOT Class QC1
Concrete, floating and finishing concrete, cutting or forming contraction joints, placing truncated domes,
application of concrete cure and seal, removing forms, repairing any seeded areas, removing of all debris,
sweeping project area, and removing traffic control.
Due to weather, or traffic concerns, the concrete may be ordered as medium, or fast set, to minimize the
closure of traffic to streets, or drives, if approved by the City of Wadsworth, prior to ordering the concrete.
Class QC1 Concrete, Aggregate Base, Truncated domes made of 24” x 24” cast iron, and
Miscellaneous items such as forming materials, anchor bolts, expansion joint material, curing and
sealing compound, topsoil, seed, fertilizer, etc shall be the responsibility of the contractor.
The contractor may bid on any of the concrete replacement items, including; both 6" and 8" concrete
pavement replacement and 4" and 6" concrete sidewalk replacement. In addition, the contractor may bid
on replacement of 6” ODOT 609 Type 2A Integral Concrete Curb for use where concrete pavement
sections that need replaced have integral curb and 6” x 18” ODOT 609 Type 6 Concrete Curb for areas
adjacent to asphalt pavement where stand alone curbing needs replaced. The Type 2A Integral Curb will
be paid per linear feet of curb completed in addition to the square yard area for concrete pavement
adjacent to the face of the curb.
Existing handicap accessible curb ramps shall be removed as part of the sidewalk or pavement removal
and demolition process. The replacement may require some additional sawing of the existing curb and
excavation to accommodate the proper dimensions of the new handicap accessible curb ramp standard.
The new standard also requires the placement of a minimum 2 each - 24” x 24” cast iron truncated dome
sections (East Jordan Iron Works 7006 or equal), which will be supplied by the Contractor. This additional
work shall be included in the unit square foot cost of handicap accessible curb ramps replaced.
Any unsuitable subgrade beyond 3” inches will be excavated to a suitable depth as directed by the City,
and filled with compacted ODOT 304 Aggregate Base at City's option as to whether it is to be paid for
under a separate bid item/price or supplied by the City.
The cost of saw cutting shall be performed by the contractor and the cost included in the price bid for the
designated item.

Removed concrete shall be hauled to the City of Wadsworth’s Material Staging Area near the Wastewater
Treatment Plant on Weber Drive. Unsuitable subgrade shall be removed as directed by the City and
hauled by the contractor to a site chosen by the City of Wadsworth. The cost of hauling and dumping of
the concrete and unsuitable material shall be included in the unit cost of each concrete replacement item.
Payment will be made for the actual square feet of concrete pavement, apron, sidewalk, or
handicap accessible curb ramps replaced, or the actual linear feet of concrete curb, as measured
in the field.
The quantities specified in the bid are to allow the contractor to be paid a premium for small projects that
are less than a total of 500 SF to compensate for miscellaneous items such as mobilization, traffic control,
overhead, etc. that are essentially fixed costs regardless of quantity. Any project greater than a total of
500 SF will be compensated at unit price bid for that item. Project square footage will be the total square
footage of the surface of concrete being replaced, regardless of the type of concrete specified, or the
number of repair holes designated. The limits of the project will be reasonable, but may vary, and may
extend to adjoining streets. The Contractor and the City need to agree upon the limits of the project, and
the appropriate unit cost, prior to commencing the work.
The same principle will apply to lineal feet of curb at the lengths designated.
The Contractor shall provide proper Maintenance of Traffic per the Ohio Manual of Uniform Traffic
Control Devices (OMUTCD), including all signage, barricades, lights, cones, barrels, etc. Trained
flaggers shall be provided to properly direct traffic as necessary for the proposed work zone. Maintaining
the traffic control devices shall be the responsibility of the contractor throughout the duration of the
contract. Proper removal of the traffic control devices shall be performed immediately following the
completion of the work needing the devices.
It is the intent of the City of Wadsworth to minimize the duration of lane closures to minimize the
disruption of traffic, reduce the exposure of traffic to hazardous conditions, and increase safety.
Therefore, it is preferred that repairs are opened and closed the same day. Due to the size of the
projects, it may be necessary for repair holes to remain open overnight. If this is necessary, it is the intent
to let no excavated repair be open more than one night. Any hole remaining open shall be protected with
lighted and reflectorized barrels, cones and/or Type I barricades per the OMUTCD.
Once the repairs are completed, the traffic control devices must remain in place until the concrete has
properly cured, and approved by the City. Any street having repairs on both sides of the road need to
repair all holes in one direction at a time. Once the first direction of approved, the devices can be
removed and repairs may be performed in the other direction.
Maintenance of Traffic shall be paid at three rates depending on the level of control provided. The
projects will be designated as MOT Type 1, Type 2, or Type 3, as follows:
MOT Type 1 - Two lane residential, or minor collector streets with low traffic volumes: Proper lane
closures per OMUTCD. Type III barricades placed at both ends designating "Road Closed to Through
Traffic" is permissible. Proper traffic control devices placed to protect traffic hazardous conditions. For
night closures protect with signs, lights and lights and traffic control devices.
MOT Type 2 - Two lane arterial or major collector streets: Proper lane closures with signs and flaggers
per OMUCD while any lane is closed to traffic. Use medium or fast set concrete to minimize lane closures
as approved by the City. Proper traffic control devices placed to protect traffic hazardous conditions. Any
trench remaining open shall have steel plates placed prior to opening to traffic.
MOT Type 3 - Four lane arterial or major collector streets: Proper lane closures with signs, barrels, cones
and arrow boards per OMUCD. Use medium or fast set concrete to minimize lane closures as approved
by the City. Proper traffic control devices placed to protect traffic hazardous conditions. Lane may remain

closed overnight with proper signs, lights and traffic control devices. Arrow boards will be provided by the
City as needed.
The preferred separation distance between warning signs is 100' in 25 mph zones, and 200' in 35 mph
zones. The contractor must consider existing drives, sidewalks, poles, signs, vegetation, curves, crests,
etc. in the placement of signs and adjust to location appropriately.
The contractor shall be paid one Maintenance of Traffic item per project to compensate for the type of
traffic control required by the OMUTCD for the length of time required. The Type of MOT will be agreed
upon by the contractor and the City prior to the commencement of work.
ST - 1 STORM AND SANITARY SEWER TELEVISING
The contractor shall provide all labor and materials to televise various sizes of storm sewer and sanitary
sewer in the City of Wadsworth. Prices will include a full report and CD record of all footage televised.
The contractor shall perform proper identification of distresses in accordance with standard PACP coding.
Records and CD’S must be delivered to the City Of Wadsworth within 2 weeks of completion of said work
Prices to include all transportation costs, fuel surcharges, lodging costs, or extra charges billed on a per
hour rate that includes the televising as stated above. The City will provide necessary traffic control and
will provide an inspector to help locate the work and instruct the contractor of all work necessary for each
day worked.
The City may perform all jetting of lines prior to the contractor commencing the televising and recording of
the sewers. The contractor may also bid on cleaning the sewers prior to performing the televising, as a
separate cost.
All successful contractors must be fully insured and have several references that can be furnished upon
request. All minimum hours per day must be stated on said bid. Payment will be based on an hourly rate
beginning at the Wadsworth office and ending at the same office and will be approved prior to billing.
The proposal is based on an hourly rate for worked performed with a minimum of four hours, and/or
a cost per hour to perform work with a minimum 8 hours work.
ST - 2 STORM AND SANITARY SEWER JETTING
The contractor shall provide all labor and materials to jet/clean various sizes of storm sewer and sanitary
sewer in the City of Wadsworth. Prices will include a full report of all footage cleaned.
Prices to include all transportation costs, fuel surcharges, lodging costs, or extra charges billed on a per
hour rate that includes the jetting/cleaning as stated above. The City will provide necessary traffic control
and will provide an inspector to help locate the work and instruct the contractor of all work necessary for
each day worked.
All successful contractors must be fully insured and have several references that can be furnished upon
request. All minimum hours per day must be stated on said bid. Payment will be based on an hourly rate
beginning at the Wadsworth office and ending at the same office and will be approved prior to billing.
The proposal is based on an hourly rate for worked performed with a minimum of four hours, and/or
a cost per hour to perform work with a minimum 8 hours work.
E 1 EQUIPMENT AND LABOR
This proposal is to supply various pieces and types of equipment and labor for hire to the City of
Street and Water Divisions - services specifications Page 4 of 14
Wadsworth.
Cost per hour amounts listed on the Contractor's Bid Blank shall include all costs, including any permit
costs to deliver, load, unload or otherwise transport or trailer the equipment to work sites within the City of
Wadsworth. Although permits may be required, permit fees will be waived on roads under The City of
Wadsworth jurisdiction. The unit price bid will go into effect at the time requested by The City of
Wadsworth for the equipment to arrive at the work site.
Unit prices submitted shall include all costs for fuel, insurance, liability insurance, equipment
maintenance, and any other costs associated with the equipment item bid.
The Contractor shall provide general liability insurance while work is in progress and during the time the
contract is in effect. The amount of coverage shall be a Minimum of ONE MILLION DOLLARS
($1,000,000) property damage and ONE MILLION DOLLARS ($1,000,000) bodily injury.
The Contractor shall provide comprehensive automobile liability insurance for all owned, non-owned, and
hired vehicles. The amount of coverage for each occurrence shall be a Minimum of ONE MILLION
DOLLARS ($1,000,000) property damage and ONE MILLION DOLLARS ($1,000,000) bodily injury.
A Certificate of Liability Insurance must be submitted to the City of Wadsworth and made an attachment
to the successful bid prior to the execution of the contract agreement. Said Certificate of Liability
Insurance shall name the City of Wadsworth as an additional insured.
The Contractor shall be liable for all amounts including that in excess of the insurance, if any, and the
Contractor shall agree, in writing, to hold the City harmless from all claims of damages resulting from the
work undertaken.
Should Contractor’s policy of liability insurance be canceled or subject to reduction of coverage,
Contractor will immediately notify the Engineer of such cancellation or reduction and shall use its best
efforts to obtain the level of coverage required by this Contract. The inability of Contractor to obtain such
replacement coverage may warrant the immediate suspension of this contract, at the sole discretion of
the City.
Bidders may elect to submit unit prices for as many equipment types and sizes as they have available
and which are requested in these bid documents.
Bids may be awarded to multiple bidders for the purposes of equipment availability, scheduling and
location.
All equipment unit prices submitted for this bid are to be without operator costs. Labor costs for
operators, truck drivers, laborers and flaggers are to be submitted separately.
Equipment Availability and Scheduling
Bids may be awarded to multiple bidders for the purposes of equipment availability and location.
The City of Wadsworth will notify successful bidders when the need for specific work arises. When
notified of the need, successful equipment bidders should respond with the availability and possible
scheduling opportunities for the needed equipment and or labor. When acceptable scheduling
arrangements can be made for each party, The City of Wadsworth will notify the bidder and be placed on
the equipment bidders' work schedule. Any changes to the agreed upon schedule by either party shall be
promptly communicated to the other party. Confirmation of availability shall be made by the bidder as the
scheduled work period approaches.
