Family Professional Partnership

The Connecticut Department of Public Health (“DPH” or “the Department”) is seeking proposals to identify one (1) Contractor who shall develop, implement, evaluate and administer regionally based medical home care coordination services through a Family Professional Partnership Outreach and Education Program that supports Children and Youth with Special Health Care Needs (CYSHCN) and providers of services for CYSHCN. A total of up to $357,000 is available to support one contract. Services are to include family/caregiver support services; outreach and education for families; access to primary care and community providers; and assistance for families with referrals to community resources, including but not limited to the CT Medical Home Initiative for Children and Youth with Special Health Care Needs. Funding will be for three years, plus one, plus one beginning July 1, 2026, through June 30, 2031, subject to the availability of funds and satisfactory program performance.
