1. Contracting Activity: Department of Veterans Affairs, Regional Procurement Office West (RPOW),
Network Contracting Office 22 (NCO-22), 4811 Airport Plaza Drive, Suite 600, Long Beach, CA 90815 as requested by VA Phoenix Healthcare System.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This
supply equipment procurement is for Stryker Open shoulder Sets and in accordance with FAR Part
11.105(a), which permits the use of brand name specifications when the particular brand is essential
to Government requirements. This equipment will be used for Veteran patients experiencing
shoulder issues requiring open procedures. Shoulder arthroplasty requires the open shoulder sets
and must be available to perform scheduled surgery cases. The purchase of these tools will satisfy
the VA standard and allow for the continuance of quality Veteran care.
Transaction amount: $21,726.87
3. Unique characteristics or circumstances that limit the requirement to "Brand Name Only''
commodities. The Stryker Open Shoulder Sets are needed to continue to address the needs of
patient care. The department currently uses two of the same Stryker Open Shoulder sets. The unique
surgical items being requested will be used interchangeably with the existing sets, and in the event
that an instrument needs to be replaced during surgery due to contamination. The open shoulder
sets are also compatible with the corresponding Stryker shoulder implant system being used in the
department. No other open shoulder sets can be used in conjunction with the current Stryker open
shoulder sets at VA New Mexico. The Howmedica/Stryker instrumentation sets included in this
request are limited to only one manufacture and there are no other substitutes.
4. Unique characteristics that limit availability to only one source, with the reason no other supplies
or services can be used: N/ A
5. Description of market research conducted and results or statement why it was not conducted:
Market research was conducted using DSBS, GSA, FPDS and the Internet websites. The research
revealed that Howmedica Osteonics Corp dba Stryker Orthopaedics has a SAC contract
#36C10G24D0041. This procurement for Open Shoulder Sets has mandatory purchasing guidelines
listed in the Surgical Implants Next Generation Ordering Guide and in accordance with FAR Part
6.302-1 entitled "Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency
Requirements", and IAW FAR 6.305. Howmedica Osteonics Corp is listed on SAM.gov and is not on
the Exclusion parties list. Howmedica Osteonics dba Stryker Orthopaedics is the only vendor capable
of providing the open shoulder sets included in this requisition request.
