Project Description: Issuing Procurement Unit: State of Utah Division of Purchasing Conducting Procurement Unit: The Utah Department of Health and Human Services (DHHS), Division of Integrated Healthcare (DIH)
The purpose of this RFP is to enter into a contract to provide: consulting services to support the planning and implementation of the Reentry Initiative Section 1115 Demonstration and related requirements under the 2023 Consolidated Appropriations Act (CAA) over a 5-year contracting period. .
It is anticipated that this RFP will result in a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code .
The methodology or formula that will be used to determine the number of contract awards is: The State will use the following cost formula: The points assigned to each offerors cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. The formula to compute the points is: Cost Points x (Lowest Proposed Price/ Proposed Price).
Length of Contract: 5 Years.
Vendor must guarantee its pricing for at least one year.
