The Iowa Department of Health and Human Services is seeking Psychiatric Residential Treatment Program(s) (PRTF), which Iowa refers to as Psychiatric Medical Institutions for Children (PMICs), willing to serve young Iowans with high acuity behavioral health needs and/or co-occurring diagnoses.
Specifically, the Agency is seeking new PMICs or current PMICs to provide additional or enhanced specialized care to children including, but not limited to, those with problematic sexualized behaviors, a history of aggression, a cooccurring intellectual disability, and/or a cooccurring Autism Spectrum Disorder (ASD) diagnosis.

RFP Purpose.
The purpose of this Request for Proposal is to identify Psychiatric Residential Treatment Program(s)
(PRTF), which Iowa refers to as Psychiatric Medical Institutions for Children (PMICs), willing to serve
young Iowans with high acuity behavioral health needs and/or co-occurring diagnoses. Specifically, the
Agency is seeking new PMICs or current PMICs to provide additional or enhanced specialized care to
children including, but not limited to, those with problematic sexualized behaviors, a history of
aggression, a cooccurring intellectual disability, and/or a cooccurring Autism Spectrum Disorder (ASD)
diagnosis.
Iowa has a need for PMICs that can serve children with complex care needs within Iowa and support
the families to place an active role in treatment.
This will include the provision of ongoing support and wrap around services such as day treatment and
specialized therapy. These services can be provided in-house or in collaboration with local providers.
The selected PMIC(s) will ensure that patients receive personalized services designed to quickly
stabilize their behaviors, address their symptoms, and facilitate their swift return to family-centered
environments. The primary goal is to equip children and their caregivers with the skills and support
needed to manage their needs safely within the community, enabling children to succeed at home, in
school, and in future employment. Furthermore, the Agency aims for PMICs to contribute to Iowa's
transition toward a treatment landscape that:
Prioritizes short-term treatment with the goal of rapidly reunifying children with their families.
Widens access to treatment for all who need it, enabling all children and youth to access
services, regardless of their current involvement with child welfare or juvenile justice systems.
Reduces reliance on hospital emergency departments and reduces the need for future
psychiatric hospitalization.
Provides services that are trauma informed and uses evidence-based practice to ensure the
highest quality of care and the best possible outcomes for youth and children.
Ensures treatment is available along a continuum of care that delivers tailored treatment plans
for each child according to their individual needs and at a range of different levels of intensity.
Coordinates effectively and seamlessly with key partner entities including the Case
Management, the child’s school district, family and permanency teams, and Family Wellbeing
and Protection staff collaborative care.
Cultivates strong community networks around youth and children to support long-term thriving in
community settings after discharge.
Duration of Contract.
The Agency anticipates executing one or more contracts that will have an initial 6-month contract term
with the ability to extend the contract for 5 additional 1-year terms. The Agency will have the sole
discretion to extend the contracts.
Bidder Eligibility Requirements.
Psychiatric Medical Institutions for Children (PMICs) are eligible to participate if they meet all the
following conditions to operate in Iowa:
1. Be accredited by a federally recognized accrediting organization, such as:
a. The Joint Commission on the Accreditation of Health Care Organization
b. The Commission on Accreditation of Rehabilitation Facilities
c. The Council on Accreditation of Services for Families and Children
d. Any other organization with comparable standards
2. Have or be in the process of obtaining a license issued by the Iowa Department of
Inspections, Appeals, and Licensing.

3. Have received written approval of need from the Iowa Department of Health and Human
Services, Division of Adult, Children and Family Services.
4. Be in compliance with all applicable State rules and standards regarding the operation of
comprehensive residential facilities for children.
5. Be an approved Medicaid provider by contract start date.
6. Facilities providing outpatient day treatment for children or adolescents require approval
from the Department of Inspections, Appeals, and Licensing.
Licensure Requirement
The Contractor shall have or obtain from the Department of Inspections, Appeals, and Licensing
(DIAL) a license for a PMIC under Iowa Code Section 135H.
If the Contractor seeks to obtain a license for a PMIC under Iowa Code Section 135H, it must meet
all of the requirements of that provision, including, but not limited to the following:
A. The Contractor must currently have or be in the process of obtaining a license issued by the
Department of Health and Human Services for one of the following:
1. A comprehensive residential care facility for children under Iowa Code Section 237,
subsection 2 or
2. A substance use disorder program under Iowa Code Section 125.13.
B. DIAL has determined that the Contractor’s medical facilities and staff are capable,
knowledgeable and proficient to deliver the care and services required of a psychiatric
institution.
C. The Contractor is accredited by one of the following:
1. Joint Commission on the Accreditation of Health Care Organizations,
2. Commission on Accreditation of Rehabilitation Facilities,
3. Council on Accreditation of Services for Families and Children, or
4. Any other recognized accrediting organization with comparable standards acceptable under
federal regulation.
D. The Contractor complies with applicable state rules and standards for a psychiatric institution
adopted by DIAL in accordance with the requirements of 42 C.F.R. sections 441.150 – 441.156.
E. The Department of Inspections, Appeals, and Licensing has submitted written approval of the
application based on the Department’s determination of need in accordance with Iowa Code
section 135H.6.
F. The proposed PMIC must fulfill one of the two following requirements:
1. The proposed entity is under the direction of an agency that has operated for three years as
a comprehensive residential facility for children licensed in Iowa under Iowa Code section
237.2 or
2. The proposed entity is under the direction of an agency that has operated for three years
providing Psychiatric Services exclusively to children or adolescents (in Iowa or another
state) and the entity meets or exceeds the requirements for licensure under section 237.3.
G. The Contractor does not require court proceedings to be initiated, termination of parental rights,
transfer of legal custody for a child who has an emotional, behavioral, or mental health disorder
as more specifically stated in the requirements of Iowa Code chapter135H.6
H. The Contractor meets all of the licensure requirements set forth at Iowa Administrative Code
481, Chapter 41.

Section 1 Background and Scope of Work
1.1 Background.
The Iowa Department of Health and Human Services (HHS) is the single State entity responsible for
administering the Medicaid program in Iowa. The Iowa Medicaid Program reimburses providers for
delivery of services to eligible Medicaid recipients under the authority of Title XIX of the Social Security
Act through enrolled providers and health plans. The Agency operates this program through its
Medicaid division. The Medicaid division also operates the Children’s Health Insurance Program (CHIP)
– the separate CHIP program is called Healthy and Well Kids in Iowa, or Hawki). The mission of Iowa
Medicaid is: Iowa Medicaid is committed to ensuring that all members have equitable access to high
quality services that promote dignity, barriers are removed to increase health engagement, and whole
person is improved across populations.
Iowa Medicaid operates under a managed care system. Most services are included in this statewide
managed care structure, including long-term services and supports (LTSS), behavioral health, dental,
and pharmacy. Approximately 96% of all Iowa Medicaid Members are enrolled in a Managed Care
Organization (MCO) with 4% remaining in Fee for Service (FFS). Iowa’s Hawki population is served by
the same Medicaid MCOs and included in the total MCO population.
The State of Iowa is dedicated to ensuring Iowa’s children with behavioral health needs can access a
full continuum of behavioral health services, including residential treatment when clinically appropriate.
Residential treatment should not be utilized as long-term placement. A patient’s individual treatment
plan should drive all interventions utilized throughout the course of treatment with the end goal of the
child’s return home.
Target Population Information
Currently, Iowa has 8 PMICs, licensed for a total of 399 beds. Not all beds are in use due in large part
to staffing challenges.
