* Qualifications received after this date/time may not be accepted.
* Qualifications must be submitted on the standard forms included within the specifications
and as required by the specifications.
* Qualifications must be typed or clearly printed in ink and signed.
* Please do not submit binders, folders or advertising materials.
* No company/firm shall modify, withdraw, or cancel its qualifications or any part thereof
for ninety (90) days after the time designated for the receipt of qualifications in the
advertisement or Request for Qualifications.
* Packets must contain completed copies of all forms including the Contractors
Affidavit and Agreement, Reference Contact Information, and Sex Offender
Acknowledgement Form.

RFQ Specifications
Occupational and Physical Therapy Services
The Liberty County School System is pursuing a contract with one or more vendors to
provide OT/PT services which will assist in the evaluation, identification and therapy to
students in the district. Our preference is in person OT/PT but tele-therapists will be
considered. Based on past services, one PT and one OT is needed but this may be subject
to change based on identified students.
1. Specifications
Detailed specifications, qualifications, and scope of work/service for this RFQ are
annexed hereto as "Schedule A" and incorporated by reference as part of these
specifications as if fully set forth herein.
2. Evaluation Criteria
The following, as appropriate to individual circumstances, shall be used as criteria for
evaluating qualifications:

3. Evaluation, Review and Selection Process
An evaluation team will review all qualifications to determine if they satisfy the RFQ
requirements and specifications, determine if a qualification should be rejected and
evaluate the qualifications based upon the evaluation criteria. The highest ranking
respondent will then be recommended to the board for award of the contract, based on
price and other factors.
The qualifications will be evaluated for general compliance with notices, instructions
and specifications set forth in the RFQ. Non-compliance with any such notices,
instructions, and specifications shall be grounds for disqualification of qualifications.
4. RFQ Certification
The undersigned hereby certifies that these qualifications are submitted in good faith.
The undersigned is aware that if any of the foregoing statements made by the
undersigned are willfully false, the undersigned is subject to punishment.

Scope of Services
Occupational Therapy and Physical Therapy Services
The Liberty County School System Board of Education desires to appoint a firm(s) to provide Occupational and/or
Physical Therapy services to the Board. It is the Board's intent to maintain or improve upon the current levels of
credentials, skills and knowledge, expand the current length of the workday and work year, increase the current level
of accountability for the performance of services, and significantly reduce the near-, mid-, and long-term costs of
said services. Contracts may be awarded to different firms based upon services rendered and specific qualifications
of individual firms responding.
Occupational Therapist:
Qualifications:
* Current State of Georgia Occupational Therapy License
* Bachelor's Degree or higher in Occupational Therapy from an accredited program
* A minimum of three years experience as a licensed OT
* Valid Driver's License
Reports to:
* Executive Director/Director/Program Specialist-Department of Exceptional Learning
Performance Responsibilities:
* Responsible for the screening and/or evaluation of students referred for testing
* Conducts therapy sessions in accordance with their Individual Education Plan (IEP) services
* Contributes in the development of the IEP for students receiving and/or referred for Occupational Therapy
services
* Consults with parents, students, and school personnel to facilitate appropriate services for students
* Maintains progress monitoring records for each student receiving OT services
* Maintains and submits other reports as needed to the Department of Exceptional Learning
* Maintains medical records as needed
* Assists with the proper placement and training of equipment needed
* Performs other duties as assigned
Physical Therapist
Qualifications:
* Current State of Georgia Physical Therapy License
* Bachelor's degree or higher in Physical Therapy from an accredited program
* A minimum of 3 years experience as a licensed PT
* Valid Driver's License
