SUNY Stony Brook University Hospital intends to purchase a Digital Pathology Scanner and Related Products from Premier contract #PP-LA-615 pursuant to its authority

under section 163 (3) (vi), which authorizes the purchases of commodities from a

consortium or comparable entity in lieu of using a centralized contract and

without a formal competitive process when justified by price. Vendors who sell

equivalent Ventilator and Related Products may, if interested, contact Michael

Dillon to discuss the procurement.

This procurement is subject to NYS Finance Law §139 j&k, AKA the Lobbying Law. Therefore, vendors are cautioned that the only permissible contact for this procurement is the individual listed below. Failure to comply with this requirement can range from no contract to debarment from sales to any NYS Agency.

The Hospital recommends that vendors file the required Vendor Responsibility Questionnaire online via the New York State VendRep System. To enroll in and use the New York State VendRep System, see the VendRep System Instructions available at http://www.osc.state.ny.us/vendrep/vendor_index.htm or go directly to the VendRep System online at https://portal.osc.state.ny.us .

Vendors must provide their New York State Vendor Identification Number when enrolling. To request assignment of a Vendor ID or for VendRep System assistance, contact the Office of the State Comptroller’s Help Desk at 866-370-4672 or 518-408-4672 or by email at ciohelpdesk@osc.state.ny.us .

Vendors opting to complete and submit a paper questionnaire can obtain the appropriate questionnaire from the VendRep website www.osc.state.ny.us/vendrep or the Office of the State Comptroller’s Help Desk for a copy of the paper form.

Successful contractor shall be required to submit valid and current NYS Workman’s Compensation and Disability Forms. In addition the contractor will be required to complete and submit documents related to NYS Finance Law §139 j&k, and Public Officers Law.
