A. SOLICITATION/CONTRACT FORM.

Solicitation No. 75H70725Q00118 is hereby issued by the Department of Health and Human Services (DHHS), Indian Health Service (IHS), Albuquerque Area Office, who has a requirement for preventive maintenance (PM) service agreement of the Siemens DR Radiology system for the Radiology department at Zuni Comprehensive Community Health Center (ZCCHC) in Zuni, New Mexico.

This solicitation is set-aside for Indian Small Business Economic Enterprise (ISBEE) pursuant to the Buy Indian Act 25 U.S.C 47 as the first (1st) tier. If IHS, does not receive reasonably priced offers from Indian Small Business Economic Enterprise (ISBEE), then IHS will evaluate quotes and consider award to other small business concerns, as second (2nd) tiers.

The solicitation document incorporated by provisions and clauses are those in effect through Federal Acquisition Circular 2025-05. The associated North American Industrial Classification Systems (NAICS) code for this procurement is 811210 with a size standard of $34M.

4. A complete Indian Economic Enterprise Representation form

5. A sign SF1449.
