Solicitation Document Description: 1.1        Introduction
.1        This RFP is an invitation by the City of Toronto (the “City”) to prospective Suppliers to submit Bids for Employee Health & Wellness (EH&W) - Adjunct Medical Services. The purpose of this RFP is to select qualified Supplier(s) Health Care Professionals and Medical Specialists to provide comprehensive assessment of the health status of employees and adjunct service to support the EH&W program in the delivery of occupational health services and proactive disability management to City of Toronto employees, as further described in Part 3 (Requirements for Deliverables):
.2        This RFP shall be interpreted in accordance with Section 3.13 (Governing Law) and 3.14 (RFP Definitions and Interpretation).
.3        Invitation to submit a Bid, including participation in any pre-qualification, request for information or other similar process or exchange of information prior to the RFP, does not imply that a Supplier is automatically prequalified to meet the requirements of the RFP or that the factors which were examined during such process or exchange may not be re-examined or re-evaluated by the City during the consideration and selection process for this RFP. It will still be necessary for the Supplier to demonstrate its qualifications through the RFP process.
.4        The selected Supplier will be required to enter into an agreement with the City for the provision of the Deliverables. The final agreement shall be substantially in the form found in Part 2 (FORM OF AGREEMENT). It is the City’s intention to enter into an agreement with only one (1) or more legal entities. The term of the agreement is to be for a period of five (5) years, with an option in favour of the City to extend the agreement on the same terms and conditions for an additional term of up to two (2) additional separate one (1) year periods. The option to renew for the two (2) additional one (1) year periods is at the discretion of the City and will be subject to budget availability and Supplier’s performance.
.5        It is the intention of the City to award to one or more contracts to the highest overall Supplier who meets the requirements listed in each category of the Request for Proposal based on the initial contract period. The categories of services requested include:
.1        Category 1: One (1) Occupational Health Physician – EH&W On-Site/Virtual;
.2        Category 2: One (1) Occupational Health Physician – Toronto Fire Service (TFS) Related Medical On-site/Virtual;
.3        Category 3: One (1) Consultant Psychiatrist – On-site/Virtual
.4        Category 4: One (1) Consultant Physiotherapy/Occupational Therapy – On-site/Virtual/Off-site with availability within the GTA;
.5        Category 5: One (1) or up to two (2) Supplier(s) for Physical Functional Abilities Evaluations (FAE);
.6        Category 6: One (1) or up to two (2) Supplier(s) for Behavioral/Cognitive Functional Abilities Evaluations (FAE);
.7        Category 7: One (1) or up to two (2) Supplier(s) for Independent Medical Evaluations; and,
.8        Category 8: One (1) or two (2) Supplier(s) for Psychiatric Independent Medical Evaluations.
.6        Suppliers may bid on one or more service categories aligned with their area of expertise. Each category requires a complete proposal that fully meets requirements of the that category and will be evaluated, awarded and contracted separately. If Suppliers bid on multiple categories, Suppliers can consolidate all categories into one proposal document; however, they must clearly indicate on the front-page which categories they are bidding are being proposed.
.7        For Category 5-8, the City may award the contracts to up to two (2) Suppliers, with each receiving up to fifty percent of the total valued on final evaluation scores. For clarity, the percentage allocations will be based on the total contract value, not on the individual bid prices submitted by Suppliers. However, the City reserves the right to award to only one Supplier if the second highest-ranked Supplier does not meet requirements or is otherwise not in the City's best interest. For all categories, the City, in its sole discretion, may choose to award some, all, or none of the services proposed, regardless of how many categories a Supplier bids on.
Specific Conditions for Procurement: A Mandatory Technical Requirement is a minimum need that must be met by the Suppliers. The City will eliminate from the evaluation process any Supplier not fulfilling the mandatory technical requirements. Failure to meet mandatory technical requirements is grounds for a submission to be declared non-compliant and not considered in the evaluation process.

The Suppliers shall include documentation in their Response demonstrating that they meet the mandatory technical requirements in their Response submission.
The individual(s) being proposed must:

•        Possess the applicable professional designations related to their field; and
•        Be license or approved by their professional college or regulatory body; and
•        A member in good standing as described in each applicable Section, Scope of Work in Part 3-Requirements for Deliverables.

The City reserves the right to confirm possession and licensing through the appropriate professional college or regulatory body prior to evaluation of the submissions.
