Conducting Procurement Unit: DHHS - Correctional Health Service (CHS)
The purpose of this RFP is to enter into a contract to provide: The purpose of this contract is to provide infectious medical waste removal, transportation, and disposal services at the DHHS Central Utah Correctional Facility.
It is anticipated that this RFP will result in a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code.
The methodology or formula that will be used to determine a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code, which receives a score of at least 60% of all technical points, will receive a contract. There are 500 technical points available and 55.60 cost points. Thus, the highest scoring vendor that achieves a minimum score of 300 out of 500 technical points will receive a contract.
Length of Contract: 5 years.
Vendor must guarantee its pricing for 5 years.
