Ohio Department of Health and The Bureau of Maternal Child and Family Health is seeking an opportunity to partner with a community organization that has an established peer mentorship program to improve community engagement and reach underserved populations in Ohio.

Offerors will need to review the attached RDQ DOH 58046 for Family 2 Family Partnership #SRC0000032571 . Each Offeror must submit a technical proposal and cost form/proposal as part of its total Proposal before the closing date and time, which is 08/07 /2025 at 3PM. If no qualifed bids are received, ODH reserves the right to post additional rounds to allow more time for qualified bidders to submit proposals.

The Offeror must title the electronic files with “DOH RDQ #SRC0000032571 /DOH58046 -- Technical Proposal” and "DOH RDQ #SRC0000032571 /DOH58046 - - Cost Form_Proposal” as appropriate.

Please note there is not a page minimum or maximum for the technical proposal and bidders should refer to technical evaluation criterion on the RDQ document for guidance.  The cost proposal should include the completed Excel Cost Proposal Form.
