Description

Please furnish quotation to supply the materials and/or services indicated on page two (2), review the applicable documentation, and return any required documents to jmangis@anl.gov.

The following applicable documentation is attached hereto and made a part hereof:




  1. Appendix “A”-71COM, Terms and Conditions for Commercial Items andServices, ANL-71COM, dated February 13, 2023. Review and Accept

  2. High Risk On-Site Supplemental Conditions, ANL-366H, dated October 1,2024. Review and Accept

  3. High-Risk Job Safety Analysis (JSA), ANL-209H, dated August 7, 2024. Complete and Return



Offeror to furnish a published price list to support quoted pricing.

The attached RFP/RFQ and only Argonne Terms and Conditions will govern any resultant order.



Instructions for High-Risk Service on the Argonne Site

The following must be completed and returned for the instructions:




  1. Completed High-Risk Job Safety Analysis Form, ANL-209H, is to be sent to the Argonne Procurement Representative, Jazlyn Mangis viae-mail at jmangis@anl.gov.

  2. Insurance Certificate as noted in Section Three (3) of Form ANL-366H is to be sent to the Buyer, Jazlyn Mangis via e-mail atjmangis@anl.gov and shall provide by appropriate language that The UChicago Argonne, LLC, The University of Chicago and the USGovernment are additional insureds; that the insurance afforded by such policies is primary insurance and, that all rights of the insurer forcontribution from other insurers of The UChicago Argonne, LLC, The University of Chicago and the US Government are waived. Pleasemake sure the Certificate Holder name is UChicago Argonne, LLC operator of Argonne National Laboratory.



No on-site work shall begin until a “Notice to Proceed” is received by the contractor from the Argonne Procurement Representative.



Provide the following with your quotation:




  1. Signed Page one of the RFQ (ANL-70).

  2. Completed JSA (ANL-209H).

  3. Certificate of Insurance.

  4. Provide a published price list or a redacted invoice showing pricing for the quoted, to prove price reasonableness.

  5. Are there any available discounts for DOE or does this order fall under GSA contract pricing (if yes, please include contract # and expiration date)?
