Solicitation Summary
The Maryland Department of Health (MDH) Office of Enterprise Technology (OET) is seeking a vendor to provide 12 months of support and maintenance for a Civica MultiVue License (serial: LSM0732MDM) for the term of November 1, 2026 through October 31, 2027. This is a renewal of an existing subscription.
This publication is Round 2 of the solicitation which revises the IFB with Addendum I (removal of Data Usage Agreement requirement).

BID
CONTENT
The bidder shall submit an original bid containing all required information listed in the bid content section of the IFB PDF. The bid shall be submitted on the supplied Attachment No. I Contract Bid Form . Attachment No. I Contract Bid Form, cannot be altered in any way and is to contain only the price or prices stipulated on the form. Additionally, the bidder shall submit a Letter of Authorization (LOA) and W9 with their bid submission. Failure to submit the LOA and W9 may result in disqualification of the bid.
