REQUEST FOR INFORMATION (RFI)



Cricothyrotomy Kits



1.0 Introduction



1.1 General. The purpose of this request is to obtain information from industry to support the government’s intent to establish a Qualified Supplier List (QSL) for the standardization of Cricothyrotomy Kits. This RFI is an invitation to submit to the Defense Health Agency (DHA) Medical Logistics (MEDLOG) Directorate Medical Materiel Enterprise Standardization Office (MMESO), technical information and all related supporting documentation for Cricothyrotomy Kits. This request is a market research effort undertaken by the government which will be used for information gathering purposes. The information received may be used for acquisition planning purposes and may help define future requirements for the subject items. This is not a request for quote or proposal.



1.2 RFI Submission Package. Vendors are requested to provide responses to this RFI via the RFI Vendor Response Worksheet. Please answer all questions and provide supporting documentation with response. Responses for each salient characteristic should be in the form of yes, no, or descriptive explanation. Additional comments may be provided for all responses.



2.0 Scope



This Cricothyrotomy Kits product line is to include sterile kits with a cuffed and flanged airway, a #10 safety scalpel, a 10cc transparent syringe, a tracheal hook (News type), a bougie and either an obturator (for a trach type airway) OR a stylet (for an endotracheal tube type airway). The scalpel blade and tracheal hook are surgical stainless steel disposable instruments. Product line products are not made with natural rubber latex.



3.0 Characteristics/Specifications





Feedback Requested: Vendors are requested to provide information about their Cricothyrotomy Kit products (See RFI Package Worksheet). If you don’t currently have the component(s) listed, please provide information on what you have available as an alternative.



3.1 Kit components (Simplified Bougie Assisted Open Surgical Technique Kit):



a. One (1) airway with a cuff, Inner Diameter (ID) of 6mm OR 7mm, with a flange on the distal end



b. One (1) obturator (if suppled airway is a trach-type tube) OR One (1) stylet (if supplied airway is an endotracheal type tube)



c. One (1) #10 safety scalpel with a full-length handle. Blade made from stainless steel surgical steel



d. One (1) gum elastic bougie, 15FR in size OR with an Inner Diameter (ID) no larger than the ID of the supplied airway



e. One (1) “News” type tracheal hook, made from stainless steel



f. One (1) 10cc transparent Luer syringe, with graduated markings on side



g. One (1) airway securing device (i.e. Strap OR Holder OR Tie), minimum width of ¼ inch.



3.2 For each required component please provide the following:



3.2.1 Manufacturer (MFR) name



3.2.2 MFR part number



3.2.3 Country of Origin (COO)



3.2.4 Detailed component description (i.e., type, style, size, ply, material, woven, non-woven, etc.)



3.2.5 If applicable, indicate which components have supply chain backlogs. Please provide the duration of any expected product availability delays.



3.3 Kit components (Field/Tactical Bougie Assisted Open Surgical Technique Kit):



3.3.1 Includes all components from 3.1 a-g and adds component (h.) nylon bag/pouch.



3.3.2 Please provide information from 3.2 for each component of 3.3.



3.4 Are Cricothyrotomy Kits packaged with all components in a peel pack? If not, please provide packaging details.



3.5 If current Cricothyrotomy Kits do not contain all listed components (see 3.1 and 3.3), are the listed components available?



3.5.1 Is there the ability to customize kit components? If yes, what is the estimated timeline to be able to provide customized kits?



3.6 Provide any information, in addition to the specific information requested, which you consider relevant to this initiative.



3.7 Please include your most current catalog that includes this product and/or components of this kit.



*Any information provided in response to this RFI will be considered and treated as unrestricted information, which can be used by the Government in any future planning or acquisitions. All information, comments, and suggestions may be incorporated into future acquisition documents, without identifying the source or obtaining prior approval from the source.*



4.0 Liability



The Government shall not be responsible or liable for any costs incurred by any party in the preparation and submission of any response to this RFI.



5.0 RFI Responses



The vendors’ responses shall include all detailed information requested in the RFI Vendor Response Worksheet and shall also include: (1) Company name and address; (2) Company POC (Name, phone number, fax number, and E-mail address); and (3) Commercial and Government Entity (CAGE) Code.





6.0 RESPONSE INFORMATION:
