GENERAL NOTES
1. All work and materials shall be in accordance with these plans, specifications, any revisions thereto, and the
rules and regulations of the Town of Bedford.
2. The Contractor is to visit the site before bidding to familiarize themselves with the field conditions and to judge
for themselves the extent and nature of the work to be done under this contract. No extra compensation will be
allowed because of the Contractor's failure to include in their bid all items and materials which they are required
to furnish in accordance with the contract documents.
2. Refer to the Specifications for safety and health requirements.
3. The Contractor shall comply with O.S.H.A. Standard 29 CFR - Part 1926.650, .651 and .652 for all excavations.
4. The Contractor shall provide the Engineer with a telephone number of the person responsible in the case of an
emergency, 24 hours a day, 7 days a week.
5. The Contractor shall submit shop drawings for all works to the Engineer. No construction shall be allowed until
the shop drawings are approved. All shop drawings submitted for this project shall be in U.S. customary units.
6. The Contractor is advised that additional "notes" will be found on subsequent sheets of the contract plans and
such "notes", while pertaining to these specific sheets they are placed on, also supplement the General Notes
listed herein.
7. No additional payment will be made for work called for by notes on the plans or in the specifications for which
no item number is indicated. The cost of such work shall be included in the prices for various items of this
contract.
8. Care shall be taken to retain natural growth and prevent damage to trees within and outside the limits of
construction, and not scheduled for removal. Any damage caused to this natural growth shall be restored at the
expense of the Contractor as directed by the Engineer.
9. The Contractor shall verify all existing and proposed elevations in the field.
10. Trenches shall not be left open overnight or unattended. At the close of each workday the Contractor shall fill or
cover trenches with steel plates to the satisfaction of the Engineer.
11. The existing pavement shall be kept clean and free of mud and debris at all times.
12. Existing storm drainage facilities shall be maintained and kept free of debris at all times.
13. Existing pavement shall be sawcut in a straight line where it is to be bounded by new pavement.
14. The pavement and subbase thicknesses noted on these plans are after compaction.
15. Fill material shall be free from organic matter, loam and frozen material. Soft or spongy areas after compaction
shall be removed, the pocket drained and refilled with select material.
16. All existing site features, including, but not limited to, pavement, curbing, grass, landscaping, piping, utility
lines, walls, signs, fencing, and structures, disturbed or damaged by construction shall be restored at the
Contractor's expense to a condition equal to or better than those currently existing and as directed by the
Engineer.
17. Disturbed grass or earth areas shall be provided with 4 inches minimum of topsoil and seeded or sodded as
described in the specifications.
18. Refuse from demolition shall become the property of the Contractor. It shall be the Contractor's responsibility to
dispose of all construction refuse legally.
UTILITY NOTES
1. The subsurface information shown hereon is not guaranteed as to accuracy or completeness. The Contractor shall
verify the location of all existing utility lines whether in public right-of-way, in easements, or in private property
prior to starting any excavation and shall be responsible for the same. The Contractor shall be responsible for
contacting the Underground Facilities Protective Organization, pursuant to N.Y.S. Law, 16 NYCRR Code 753.
2. The Contractor shall be responsible for the protection and/or relocation of all underground utilities that may be
encountered within the work limits. The cost of any work to protect or relocate utilities shall be included in the
price bid for various items in the contract.
3. The Contractor is responsible for contacting all utility companies to coordinate connection of services. The
Contractor shall also coordinate temporary metering of services if required.
WORK ZONE TRAFFIC CONTROL NOTES
1. Maintenance and protection of traffic outside the limits of construction, both pedestrian and vehicular, shall be
the responsibility of the Town of Bedford.
2. Maintenance and protection of traffic within the limits of construction, both pedestrian and vehicular, shall be
the responsibility of the Contractor.
3. Whenever the crane is on site, the Contractor shall utilize traffic flagmen in conformance with the requirements
of Section 619 of the current Standard Specifications, the Manual Of Uniform Traffic Control Devices
(MUTCD) with the New York supplement, and as ordered by the Engineer.
EROSION CONTROL NOTES
1. All erosion control measures downslope of the proposed work area shall be in place as shown on the erosion
control drawings prior to the start of construction.
2. Final grades shall be established as quickly as possible and topsoiled and seeded to provide a turf which will
stabilize slopes and prevent erosion.
3. Slopes to remain open during construction shall be protected with mulch and staked tobacco netting.
4. Catch basins and drain inlets shall be protected as shown on the erosion control drawings throughout the
construction period and until all disturbed areas are thoroughly stabilized.
5. Soil stockpiles shall not be allowed on steep slopes, in drainage swales or in wetland areas. Soil stockpiles shall
be surrounded with silt fencing and seeded.
6. Erosion control measures shall be inspected after each rainfall and shall be cleaned, repaired or replaced as
required.
7. The Engineer or a N.Y.S.D.E.C. field representative may require additional erosion control measures if deemed
appropriate to mitigate unforeseen siltation and erosion of disturbed soils.
CLEARING AND GRUBBING
1. Clearing and grubbing shall be required within the construction limits.
2. The Contractor is advised that these plans may not include all clearing that may be necessary for the construction
of the project.
3. Only trees 6 inches in diameter or greater are shown herein. Additional removal of smaller trees within the
project area shall be required.
4. Existing trees within and outside the limits of construction, and not scheduled for removal shall be preserved and
protected wherever possible and shall only be removed with the approval of the Engineer. Tree roots shall be
avoided wherever possible. Where tree roots cannot be avoided, they shall be cut cleanly. Tree roots shall be
kept moist while trenches are open. Any damage caused to this natural growth shall be restored or replaced at the
expense of the Contractor as ordered by the Engineer.

REMOVAL NOTES
1. The existing superstructure, which includes the steel beams, diaphragms, railings, and other appurtenances,
connected to or on the superstructure system shall be removed.
2. A removal plan shall be submitted to the Engineer prior to beginning the demolition.
3. Loose concrete and stones may become dislodged during removal operations or during transportation from the
site unless appropriate measures are taken. The Contractor shall formulate and submit a method of remediating
the condition for approval by the Engineer. The cost of remediating this condition shall be included in the lump
sum price bid for the superstructure removal item. The use of environmental ground and/or waterway protection
items will be required.
4. During removal operations, the Contractor shall not be allowed to drop waste concrete, debris and other material
to the area below the bridge. If the Engineer determines that adequate protective devices are not being
employed, the work shall be suspended until adequate protection is provided.
5. The cost of furnishing, installing, maintaining, removing, and disposing of all nets, screens, or other protective
devices shall be included in the unit prices bid for the appropriate items of the contract.
6. Refuse from demolition, unless noted otherwise on these plans, shall become the property of the Contractor. It
shall be the Contractor's responsibility to dispose of all construction refuse legally. Whenever items in the
contract require materials to be removed and disposed of, the cost of supplying a disposal area and
transportation to that area shall be included in the unit prices bid for those items.
STORAGE OF MATERIALS
1. The Contractor shall be responsible for obtaining permission from the Town for locations to be used to store
materials within the right-of-way. The Contractor shall obtain the written permission of the property owners
prior to utilizing any areas outside of the right-of-way to store materials. A copy of any written permission to
store materials on private property, if obtained, shall be submitted to the Engineer.
NON-PAVED SURFACE NOTES
1. The Contractor shall not expose any subgrade that cannot be backfilled and compacted with subbase material by
the end of each work day or as approved by the Engineer.
2. Compacted subbase material shall not be exposed for more than seven calendar days or as approved by the
Engineer.
SEQUENCE OF CONSTRUCTION
(The following sequence is intended only as a guide to the Contractor. The Contractor shall be solely responsible for
the Sequence of Construction.)
1. Prior to disturbance of any kind, and throughout the life of the project, the Contractor shall have a copy of these
plans, specifications, and all applicable permits on site.
2. The Contractor shall have a pre-construction meeting with the Engineer, Town of Bedford, and all other
interested agencies prior to the commencement of any work.
3. Place all traffic control maintenance devices, barricades, etc.
4. Install the silt fence and other sediment control devices.
5. Clear and grub as necessary within the limits of construction.
6. Install collection devices as necessary to catch debris during removal of existing culvert deck superstructure.
7. Remove the existing culvert deck superstructure.
8. Construct new abutment liner walls and footings.
9. Backfill excavations for abutment liner walls.
10. Stabilize disturbed areas as soon as possible.
11. Construct temporary unloading pad.
12. Install new precast concrete planking and bridge rail.
13. Remove temporary unloading pad.
14. Place new roadway subbase and asphalt pavement.
15. Establish finished grades and install topsoil and seeding.
16. Install remaining guide rail.
17. Reinstall sign and pavement striping.
18. Restore all areas used for storage, access, etc., and clean up project site.
19. Remove all traffic control maintenance devices and open to public. After all areas have reached 80%
stabilization, remove all sediment control devices.
20. Clean and close site.
