2025-2030 Physical Therapy Services in Franklin County

OPWDD's Contract Management Unit on behalf of Surmount DDSOO is soliciting sealed bids from responsible and qualified Physical Therapists to provide services to people with developmental disabilities. residing in our state operated/certified facilities located in Franklin County. Services are to be provided in Franklin County at Surmount DDSOO’s Campus at 2445 State Route 30, Tupper Lake, NY 12986, as well as adjoining locations at 380 Hosley Ave., Tupper Lake, NY 12986 and 390 Hosley Ave., Tupper Lake, NY 12986.

The Physical Therapist is to provide Physical Therapy services an estimated twenty (20) hours per week but will not exceed 800 hours per year. This agreement makes no guarantee for referrals and/or provisions of a minimum amount of services in the calendar year.

A qualified Physical Therapist shall be licensed and registered in New York State. Familiar in providing service to individuals with developmental disabilities. Familiar with adaptive equipment (e.g., seating, positioning, bathing, etc.) for severely and profoundly affected individuals. Since the Physical Therapist will have substantial contact with the individuals receiving services, pre-employment background checks may be required.

OPWDD may request bidders to submit work references that will verify that the bidder or its principals have at least three (3) years of relevant experience to complete the work as listed in Qualifications and Scope of Work. Upon request, bidders will have 3 business days to submit such references.

No Subcontracting of services is allowed with this IFB without written permission of OPWDD.

Awards will be made to the qualified bidder with the lowest Total Annual Estimated Cost for Physical Therapy Services as outlined in the Scope of Work for Sunmount DDSOO Physical Therapy Services. Prevailing Wage does not apply to this contract.
