3. Contractor must review the following table and initial next to each item to show the
Contractor understands the specific requirements and what is expected. Failure to initial
next to each item may result in rejection of bid.

ITEM
The City requires a Licensed Plumber in the State of Illinois to perform all service
1. connections.
No excavated material shall be placed on the street to block traffic. All excavated
material shall be loaded onto trucks or other means of relocating material. In cases
2. where this is not possible, Contractor shall seek prior approval of the City.
3. The project will be awarded based on the Total Base Bid Price.
Installation of the water main shall be the Contractors responsibility to locate or have
located all utilities being crossed, connected to, or otherwise impacted. It shall be
required to locate these utilities regardless of the installation method utilized for the
water main. Utilities crossed by Horizontal Directional Drilling shall also be located
and exposed by the Contractor. It shall be the Contractor’s responsibility to repair
any damaged utility and for any damage to property, private or public associated
4. with damaged utilities, during the installation of the water main.
The City requires the contractor to fill out the “Water Service Line Material Survey”
for each water service that will be connected to the new water main. This can involve
entering the house to identify the service if owner permits access. Contractor has
reviewed “Water Service Line Material Survey” as seen in Exhibit 1 of the
Specifications and on the Detail Sheet D1.

The City requests the contractor to provide project references. If references are not
6. provided the City may reject the bid.

1. The undersigned submits herewith these references for similar work completed by him
within the last 5 years. The City has permission to contact the provided entities and
reject bids based on past work with these references or the City. If references are not
provided the City may reject the bid.
