Project Description: The project involves pavement repairs on a culvert along Kamehameha Highway, Kahuku. Scop includes:
1) Cold-plane to 12 inches below grade within the affected lane and include the right shoulder lane. Cold-planing should extend to at least 12 feet from the box culvert.
2) Lay Tensar InterAx FilterGrid NXAC-FG or equal onto the exposed grade and pin it to the exposed concrete of the box culvert by flapping the fabric grid upwards and securing it with concrete screws in two rows with screws spaced 6 inches on center. The two rows of screws should be spaced 6 inches on center also.
3) Place two layers of 4.5 inches thick of Asphalt Concrete Base (ACB) layers. Total of 9 inches.
4) Cold-place 3 inches on the affected lane and right shoulder over the box culvert. There should be 4-5 inches of asphalt pavement over the box culvert based on the number of overlays placed in this area over the years based on the as-built plans.
5) Place Tensar Rapid Road Repair PG100 or equal over ACB and over cold-planed area over the box culvert. Make sure the overlay widths over strips are per manufacturer's recommendations.
6) Place 3 inches PMA Mix IV.
7) Perform final cleanup, striping restoration as required, and site restoration.
Traffic Control: Implement traffic control for lane and shoulder closures in accordance with approved Traffic Control Plans and applicable standards.
Cancellation: This solicitation may be cancelled and any or all bids rejected in whole or in part, without liability to the State, when it is determined to be in the best interest of the State.
Note: All pricing should be inclusive of all applicable taxes, fees, and charges. Attached as-built drawings are provided for information/reference purposes only.
Bond Requirements: All bidders shall be required to include a Performance Bond and a Payment Bond in accordance with HRS103-D324 upon award of the contract. These bonds shall each be in an amount equal to 100% of the contract price and shall be submitted within the time specified after award.
Insurance Requirements: Contractor shall obtain and maintain all required insurance coverage in accordance with State of Hawaii requirements. Refer to the Insurance Requirements section for full details.
