STATEMENT OF WORK
Office Furniture
The C&A requirements do not apply, and a Security Accreditation Package is not required.
GENERAL SCOPE
Building 35 Compass Treatment Team rooms are currently being modified to meet MHEOCC Checklist. The new furniture requested is required to make the rooms MHEOC compliant to use for Treatment Team.
This requirement includes new furnishings for Building 35 Compass Treatment Team rooms. If any furnishings are found to be damaged at the time of install, they are to be re-ordered and replaced in a timely manner. All products must be made and assembled in America.
Must provide product listed or can propose equal product if it matches specifications for product requested. Specifications must be submitted for all products if an or-equal product is being submitted. Furniture must be delivered to Building 35 Compass Center first floor and installed during normal business hours Monday Friday 8am-4:30pm excluding weekends and holidays. One month s storage fee included (no extra fee to the government) to hold furniture just in case the rooms are not ready.
2.0 PERFORMANCE REQUIREMENTS
The contractor shall:
Provide and be responsible for the technical assistance, development, and generation toward the final specifications.
Provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity.
Provide delivery and installation of new items.
Provide removal of associated waste product as required; contractor is to dispose of all waste associated with product and trash.
Comply with duty hours and observed US Government recognized Federal Holidays. Duty hours will be 0800 1600 Monday through Friday. Weekends times are available if needed. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Saturday, it is observed on Friday; if the holiday falls on Sunday, it is observed on Monday.
Coordinate and provide project management of all products and services to manage, design, order, ship, receive, and install new furniture from the manufacturer until final acceptance by the Contracting Officer Representative (COR)/ VA Tia Manring.
The contractor shall: Deliver and assemble all items to:

Time and date will be determined by VA Point of Contact, COR Tia Manring, after receipt of order.
Comply with requirement to ensure area is clean of any packing debris before leaving following installation.
Provide maintenance manuals for all products specified.
3.0 SUBMISSION OF QUESTIONS
No questions will be answered for this solicitation. This solicitation is a follow on to a previously issued solicitation for which an opportunity to submit questions was provided. All questions were addressed during that initial posting.
4.0 SUBMISSION OF QUOTATION
All quotes must be received by August 12 at 12:00 Eastern. Quotes must be emailed to Rachel MacRae, rachel.macrae@va.gov.
The contractors shall submit complete emailed electronic copy portable document format (.pdf) or Microsoft Excel (.xls) format of:
VOLUME 1: Technical
Technical drawings complete manufacturer s product specifications
Quantity of man-hours for labor/installation
Project management plan
Product literature
Project staffing plan
Warranty
Self-certifying statement confirming ability to meet project deadlines
VOLUME 2: Pricing
Bill of Materials tagged per CLIN
Price quote with separate line for delivery/assembly/labor/installation of all items.
Items will need to be unloaded and deployed in the facility assigned space.
Quotes submitted by any other method will not be considered. All proprietary information shall be clearly marked. The use of hyperlinks in quotes is prohibited. Late quotes will not be accepted for evaluation. The contractor is responsible for confirming the Government s receipt of the contractor s quote.

ADDITIONAL:
Integration Management Control Planning. Provide the technical and functional activities at the required level for integration of all tasks specified within this SOW. Include productivity and management methods such as quality assurance, progress/status reporting and program reviews. Provide the centralized administrative, clerical, documentation and related functions.
Contract Management. Prepare a Contract Management Plan describing the technical approach, organizational resources and management controls to be employed to meet the cost, performance and schedule requirements throughout Contract execution. Provide a monthly status report monitoring the quality assurance, progress/status reporting and program reviews applied to this contract.
5.0 EVALUATION CRITERIA
Task order will be evaluated on:
Cut sheets with product detail for each line item
Item meeting the requirements as stated below (Brand Name or Equal)
Labor/Services appropriate to the scope of work
Project staffing plan appropriate to scope of work
Project management plan appropriate to scope of work
Signed Limitations on Subcontracting form (provided and REQUIRED)
Made in America

7.0 Delivery and Installation
Delivery of all items to Chillicothe VA Medical Center, Building 35, Rooms 2B11/2B123/2B124 at 17273 State Route 104 Chillicothe, OH 45601. Time and date will be determined by VA Point of Contact, COR Tia Manring, after receipt of order. All furniture must be delivered and installed upon time of installation the VA cannot store any furniture for any reason. The furniture must be inspected for damages during installation and dealer is expected to repair/replace any damaged furniture. The installer must point out all damaged furniture to the Interior Designer and go over plan to fix and or replace the damaged furniture. It needs to be written up in final paperwork for VA to sign off on for installation completion.

Inspection and Acceptance Criteria
If any pieces of furniture are damaged and to be replaced it is the dealer s responsibility to coordinate delivery and install of the pieces with the Interior Designer. The COR (Interior Designer) will sign all necessary paperwork for installation and delivery.
8.0 Performance Monitoring
VA Interior Designer will be present for installation and will be the point of contact for deliveries and installations. If any issues arise the Interior Designer must be notified. If any pieces of furniture arrive damaged it is to be noted on final paperwork and to be replaced as needed at no cost to the VA Medical Center.
9.0 Other Pertinent Information or Special Considerations
Possible Follow-on Work If any pieces of furniture are damaged and to be replaced it is the dealer s responsibility to coordinate delivery and install of the pieces with the Interior Designer.
10.0 Place of Performance
All furniture is to be delivered and installed to the Chillicothe VA Medical Center Building 35, 17273 State Route 104 Chillicothe, OH 45601.
Building 35, first floor. There are two elevators on the ground floor. No loading dock as you enter the building at ground level and take the elevator up to the second floor.
