THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE SUPPLY CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. The Defense Health Agency Contracting Activity (DHACA), Western Markets Contracting Division (WMCD), San Diego, intends to award a firm fixed price contract for Blank Purchase Agreement (BPA) for Neonatal Total Parenteral Nutrition (Neo-TPN) and Methemoglobin Testing in support of Blood Bank Department, Naval Hospital Guam in accordance with Federal Acquisition Regulation FAR 13.106-1(b)(1)(i), only one source being available to meet the Government’s needs, to:

The neo-TPN and methemoglobin testing services are required to support the establishment of a new 3-bed Neonatal Stabilization Unit, which will provide 24/7 care for critical neonates for up to 10 days at NHG. GRMC is the sole hospital on the island of Guam that can support neonatal TPN and methemoglobin testing for neonatal critical care patients.
Neo-TPN requires highly specialized formulations tailored to the unique nutritional needs of premature or critically ill infants. These formulations often involve specific concentrations designed for critically ill neonates. Neo-TPN solutions must be custom compounded to address the specific needs of each infant based on weight, gestational age, and medical condition. This level of customization and expertise in neonatal care is solely offered at GRMC. The island’s geographical location limits the availability of suppliers who can meet the stringent requirements of neo- TPN. Shipping logistics and distance prevent alternative suppliers from providing the specialized formulations required for critically ill neonatal patients.
Methemoglobin testing requires specific equipment, reagents, and test kits available only at GRMC. The testing must be conducted on the island, and geographical isolation has limited the availability of multiple laboratory test centers. The Naval Hospital Guam (NHG) has identified GRMC as the only reliable testing center within reasonable transportation distance or logistical capability to perform the test. Methemoglobin testing and results are critical in urgent medical situations for critically ill infants, and GRMC is the sole hospital that can quickly provide the necessary services to meet immediate healthcare needs and ensure quality service.
This notice of intent is not a request for competitive proposals, and no solicitation document exists for this requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to provide the required product for United States Naval Hospital Guam. Capability statements shall not exceed six (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
